Really no stars this place is awful. Did not attend to my father at all first 2 weeks. He ended up declining. We had to beg for even a wheel chair. When he was sick we had to before for him to get medical attention and when they finally did they have him an antibiotic just to give it to him he was not examined the antibiotic was ine that was not for a man his age with the health issus he has , the side affects were more likely to happen per the reviews on this medication cause my father to decline even more. He ended up falling from the bed multiple times due to the hallucinations the medication caused and they refused to call 911 for an ambulance I had to call from out of the area. Luckily I did. He was in the er and I refused him to go back there. He lost 20 pounds in 2 weeks. He was diagnosed malnutrition and having hallucinations all due to being at this facility. My father was ready to get rehab when he got there he was able to stand with a walker now he has only declined due to this place and he is only alive because we were there every day amd on it. I wouldn't not recommend this place to anyone. This is why they always have openings in all the Aviata locations. They are all run by the same corporation so I wouldn't even try my chances with any of the other locations. I have been responded and this has been reported. I do not need a response on this review. This is only to help people in the position with a loved one. I do care about people unlike this rehab facility.

I would like to express my sincere gratitude to Nurse Director Shannon, Addmissions Crystal, Stephanie, Nurse Supvisor Cherry, Nurses Alice/ Karen/ Christine, CNA Nick, CNA Carla, CNA Marline and for their continued professionalism, treating me with the level of care and respect, that is in my opinion, above and beyond. Communication, listening to patients and taking the time to remedy any issue a patient may have is a big part of standing out, being appreciated and noticed, I thank you from the bottom of my heart and hope others will follow from the example you set . I have been here for nine months. The CNA’s are very nice. The nurse takes very well care of me and the young lady named Stephanie menhon It’s a very nice person who is always here making sure I’m OK and if there’s anything I need, she’s very kind and caring and another lady. Her name is venny Takes care of me like I’m her family also, Adam, he has been very helpful to me. I have meant lots of nice people here.

Well, I have to say this place Aviata at Evans is one of the best places to come get individual patient care. The staff there are really awesome and excellent with hand on. They really treat you like family. In my book by far one of the best from my family and I.

The staff abuses the patients by letting them sit around for hours in their own mess and not washing them properly. Then when you comfront them they like to say "well thats not my job" If you love your family member, do not let them stay here. As soon as you walk into the building the foulest smell of urine and cigarettes hits you in the face. Id see residents in the hallways with soaked pants when id walk to my grandmother's room, the staff neglected her, id visit every day and it was always the same thing she was soaking wet so id have to go and complain but nothing ever got resolved because it would just happen again. At one point I even paid a staff member who more than gladly took my money, to changer her diaper in hopes shed get better care for the rest of the day and 5 hours later another family member would visit her and say shed not been checked on and her sheets needed changed. they kept "losing" her clothes because they were to lazy to seperate everyones laundry and there were days id go to visit and id see a woman wearing my grandmother's clothes! They absolutely refused to give her a shower during her stay, they said we already "washed her up" but she had a smell from laying in soaked sheets all day and her hair was oily from not being washed for weeks, I begged them to put her in a shower and they kept saying we will get to it and they never did. I even had a meeting with a woman there who said "we will open up an investigation" but that was a joke. If I could give negative starts I would. I have started the process of filing a formal complaint to the board of health on how disgusting, neglectful and poorly managed this facility is. I hope one day all the woman on staff there that neglected my gramdmother gets the exact same treatment when they are her age and find themselves in a facility like theirs, and trust me you all know who you are.
