Skilled Nursing Facility - Lee Memorial Hospital

Lee Memorial Hospital Skilled Nursing Unit is an 18-bed skilled nursing facility located on the third floor of Lee Memorial Hospital in Fort Myers, Florida. The unit provides post-acute care and rehabilitation for patients recovering from joint surgery, stroke, heart surgery, COPD, and other medical conditions, with the goal of helping patients return home as quickly and safely as possible.

The care team includes registered nurses, certified nursing assistants, physical therapists, occupational therapists, speech therapists, a social worker, and registered dietitians. Specialized services focus on balance management, stroke and cardiac rehabilitation, joint rehabilitation, and dysphagia treatment. The team develops personalized, patient-centered rehabilitation plans and utilizes the latest clinical and rehabilitative treatments to support each patient's transition to the next level of care.

The facility features a newly upgraded environment with a dining room that promotes social interaction, a therapy gym open seven days a week, a community room for patients and families, wireless internet in all rooms, an in-house pharmacy, and on-demand dining with a hospitality bar and cafeteria. Soon after admission, the care team meets with patients to define goals, discuss potential barriers to progress, and create a customized care transition plan.

As part of Lee Health, the Skilled Nursing Unit works collaboratively with patients' healthcare providers to ensure quality, safety, and continuity of care during the transition from hospitalization to post-acute care.
