York Rehabilitation & Nursing Center is a skilled nursing and long-term care facility located in Philadelphia, Pennsylvania, serving 237 reviews with a 3.8 out of 5 rating on Google.
Family experiences at York are notably mixed. Several families report meaningful individual moments, one noted that a nurse who admitted their loved one remembered their visit and greeted them warmly, and some highlight staff who maintain high standards of personal care. A long-term guardian reports that during unannounced visits over seven years, their elderly client is consistently up, dressed neatly, and receives detailed nursing updates. Residents themselves have praised the facility's openness to suggestions, noting improved food quality and willingness to implement resident ideas.
However, other families describe serious concerns including delayed response to call buttons, unmet medication schedules, unclean rooms, and inadequate wound care. One family wrote their loved one "had the call line open for over an hour" with an open wound and clogged toilet. Multiple reviews cite inconsistent staff responsiveness and professionalism on different shifts. A bereaved family noted their father's prescribed medical cream for bed sores remained unused in his closet for months.
The rating of 3.8 across 237 reviews reflects this stark divide: roughly equal numbers of five-star endorsements and one-star critiques, with some four-star accounts describing overall kindness despite limitations. Prospective families should expect variable experiences depending on shift and department.

Staff was instantly responsive to my request for patient expressed desire for liquid refreshments. It’s the little things that go along way with a rehabilitating person journey back to full health and independence. I have no reason to expect that type of responsiveness is not executed, at the patient’s request, even when no family member is present.
