Make sure your loved one receives an information packet when they are admitted. Our family member was disoriented and had no idea where they were, nor did they receive written papers or a welcome packet until we insisted on them. Also, response times to the call buttons were extremely slow and we had to push hard for support at times. On the plus side, the exterior of the building is pleasing to the eye and there were some engaging activities, esp for the longterm residents. Finally, be aware that if your loved one's stay converts from rehab (often covered by insurance) to nursing care (not necessarily covered), that the cost is extremely high, though this seems to be the nature of the industry.

My mother was a resident at this facility from April 1, 2026 until her passing on June 8, 2026. We had already made the difficult decision to remove her and bring her home because we were deeply concerned about the quality of care and supervision she was receiving. During her stay, we repeatedly experienced a lack of monitoring and responsiveness from staff. Her call bell did not work and, despite our requests, it was never fixed. She was given a handheld bell instead, but calls for assistance often went unanswered. We also observed staff appearing disengaged, including watching television on computers and sleeping during overnight shifts. We were told that previous problems were under prior management and that improvements were being made, but our experience did not reflect that. After my mother's death, we have encountered significant difficulty obtaining her medical records and getting our calls returned. Our family has to live with the decision to place our mother here. Based on our experience, I encourage families to thoroughly research this facility and consider all options before placing a loved one in its care.u

My cousin was admitted to Woburn Rehab & Nursing Center a few months ago. Thank God for the staff who went out of their way to make my cousin's transition go smoothly. It can be a confusing time for families and residents but Amy and the staff helped us step by step. We had confusing "paper work" to prepare but they helped us get it done. They keep us apprised on my cousin's activities and how she is becoming acclimated to her surroundings. We are very happy with this facility and all the staff. I would highly recommend this facility. It's in a great location, very clean, beautiful areas and grounds. Very accommodating to the needs of family and residents. Thank you all for confirming we made the best decision.

Some of the folks at Woburn Nursing were responsive and compassionate to my dad during the last two years of his life there. For example, the weekend supervisor (Sofyia) and the transportation coordinator (Luis?) really listened to his needs and went out of their way to take care of him. The reason though for only two stars in this review is because much of the rest of his experience was needlessly harsh. There was significant disregard for patients and their families on a regular basis, to the point where my father and family members had to repeatedly ask for the same things and escalate them, often to be ignored or talked to in a condescending way. Things like falling and not being discovered for a long time, being picked up and handled in a rough/painful way, being scolded after falling as if he was a disobedient child who wanted to fall, not responding to calls for help to the point where he wasn't toileted regularly and had to lie in soiled clothes, knowingly having a call button that didn't work for weeks so that he couldn't get help when needed. There was really so much that was needlessly unkind treatment that I can't list it all in a short review, but things like not providing a variety of food especially at end of life that tasted at least ok was completely preventable. He literally was given the same meal (chicken, mashed potatoes and carrots) every day for most of his time there. The lack of variety was bad enough, but I tried it, and as a non-picky eater, I found it hard to stomach. I even had to arrange his transportation to medical visits because they refused to schedule him on time, to the point where he missed his doctor's appointments entirely. Even end of life was not what it should have been. A week before he passed, he was propped up in a wheelchair at the nurse's station, convulsing and gasping for air instead of being treated like someone who was dying and in need of comfort measures. He had a plan for coming off of life support that was repeatedly ignored, which extended his suffering by weeks. Even at the very end, the overnight nurse said she could tell he was about to pass, but the family never received a call even though we said repeatedly we wanted to be there. As a result, he died alone. I hope someone will read this so that changes are made to how people are treated. This experience wasn't necessary and wasn't right.

Only send your loved ones here if you want them to die. There is some serious patient negligence here. My grandmother was sent here for what should have been a short stay after being in Lahey. Within the first two weeks of her being here they called me telling me I should switch her to Medicare even though her insurance covered 100 days in a rehab- a smart ploy that would give more money to them. One night after returning from her dialysis session no one answered the phone to let us back into the building and we stood outside not knowing what to do for 30 minutes until a maintenance worker gave me the code to get into the building- a serious safety concern. When finally making our way back to her unit they told me no one is paid to answer the phone and the woman at the desk said it wasn’t her job to do so. We asked multiple times how she was doing, how her therapy was going, etc, and they barely touched her. They just let her rot in the bed. The patient in the bed next to her would ask the nurse constantly when the therapist would be coming. One night we found a mouse in her room, trapped it in a container, and by the time maintenance came (30 minutes later) mouse had escaped. While in the rehab my grandmother got the flu and I had to ask that she be tested for Covid and the flu to determine that she had it to get her started on treatment right away. If I was not actively involved in her care nothing would have been done. This facility does not care about patients at all, and there are no standards in place or metrics that need to be met in order for your loved one to get out of here. If you don’t call, or aren’t there constantly, no one will reach out. My grandmother seriously declined under their care. She went in walking and is now bed bound with bed sores. The PT, social worker, nurses, everyone will sweet talk you and say everything is fine to keep them there and continue to claim their paycheck. There was a consistent pattern of neglect, poor communication, and indifference over weeks. If you are not able to be physically present, advocate aggressively, and monitor every aspect of care yourself, your loved one will not receive appropriate treatment here, and they probably still want even if you can be constantly present. Do not send someone you care about here. This is by far the worst rehab I’ve ever experienced.
