Windsor Care Center of Sacramento is one of only two facilities in Sacramento, California that specializes in offering secured alzheimer’s and dementia care. We specialize in offering care, rehabilitation, and skilled nursing to our residents who suffer with progressive stages of memory loss. We strive to help our residents increase their independence and reach their highest level of functionality.

In an effort to make as many residents feel at home as possible, our nursing staff is fluent in several languages. At least one nurse speaks Spanish, Tagalog, Korean, Igbo, Polish and Farsi.

Our dedicated alzheimer’s and dementia rehabilitation team work with some of the most complex post-surgical care needs have helped countless severe stroke patients restore to their highest level of function. Patient Centered Care

At Windsor Care Center of Sacramento, we offer a full range of Nursing Services to our alzheimer’s and dementia care residents including: 24-hour skilled nursing care, pain management, diabetes management, dietary programs and much more. Our facility is unique in the fact that all of our residents are dementia and alzheimer’s patients. Our team is specially trained in working with progressive stages of memory loss, and we work with each resident to individualize their care plan.

Rehabilitation is a primary concern for all of us at Windsor. We believe that the more independent you feel the happier you are. Our physical therapists develop a program based on the individual needs of each alzheimer’s and dementia care resident. Our occupational therapists assist with activities of daily living (ADL) to help gain independence. Stroke, brain injuries, or respiratory illnesses can sometimes affect swallowing and ones ability to communicate. Our speech therapists work to help you regain as much independence as possible.

Often, residents arrive here thinking they just need to improve their physical capabilities. Once here you realize the happy environment improves your outlook on life. We employ energetic and friendly team members to ensure there are always Activities in which to participate. You’ll surely gain new friends in the staff and the other residents while participating in games, outings, and performances. Additionally, our Social Services team proactively helps with appointments, transportation, and discharge planning.

Secured Alzheimer’s and Dementia Care

Windsor Care Center of Sacramento is a leader in Secured Alzheimer’s and Dementia care. We are one of only two facilities in our area that specializes in dementia and alzheimer’s care.

Our staff is compassionate, and we understand that when someone is suffering with the progressive stages of dementia they will need support and assistance. We offer the care needed to help with this process. We offer services such as walking together, reminders for eating and drinking, help with managing sleep behavior, rehabilitation services, services to help as disease advances, and, most importantly, Windsor’s Personal Touch.

As a loved one is suffering from dementia and other related illnesses, it can be a taxing time for family members. We are there to help relieve those family caregivers. Using respite care can help you feel rested and not overburdened to enable you to be a better caregiver for your loved one.

Our knowledgeable and caring staff provide specific programs and purposeful activities in our secured unit tailored to the individual needs of your loved one. Nursing Services

Caring and Healing

Our center is unique because the nursing services we provide are customized for those suffering with dementia or Alzheimer’s. We provide a high level of medical and nursing care. In an atmosphere that is appreciated by both residents and their families, physicians and other highly-skilled professionals monitor each patient.  Nurses and Certified Nursing Assistants provide skilled care as well as assistance with getting in and out of bed, feeding, bathing and dressing. Diabetes Management Our experienced nurses provide help with all aspects of managing diabetes including: blood glucose monitoring, insulin administration and tracking, exercise plan, menu planning, education, and more.

Dietary Program We provide meals to meet specific dietary and therapeutic needs, as directed by physicians. We also offer oral supplemental diets. There are regular nutritional assessments from our registered dietitian. Windsor’s skilled nursing staff provides compassionate, patient-centered care and is genuinely committed to returning patients to their lives as safely as possible.

Rehabilitation Services

Bridging the Gap to Home

Our rehab team is uniquely set up to work with those who suffer from progressive stages of memory loss. We work with your loved one to reach their health goals as quickly and efficiently as possible. We partner with physical, occupational, and speech therapists to achieve these aspirations of independence. We provide state-of-the-art equipment and highly trained staff to achieve the balance of making this possible. Windsor’s ultimate goal is to safely return residents to their homes, or other appropriate living arrangements, as soon as possible. While each resident’s therapy regimen is prescribed by their physicians, progress towards goals is assessed weekly in order to monitor the treatment plan to ensure clinical needs are met and/or exceeded.

Physical Therapy

Our physical therapists focus on identification and rehabilitation of individuals with physical impairment and pain, as well as future injury prevention. Our physical therapy team’s goal is to develop a program that is based on individualized needs. They focus on the specific needs that arise for those with dementia and alzheimer’s.

Occupational Therapy

Our occupational therapists work with residents in adapting to their social and physical environment by working on tasks essential for daily living. An occupational program may include working on functional independence for things such as cooking, eating, bathing, dressing, and grooming. Our therapists also work with residents in increasing safety awareness for when residents return home.

Speech and Language Pathology

Our speech and language therapists work with residents affected by stroke, brain injury, dementia, hearing impairment, respiratory illnesses, and other complex medical conditions. Our speech therapy program is designed to improve speech, increase comprehension and verbal expression, improve chewing and swallowing, and maximize cognitive skills.

We offer numerous activities that allow residents to associate with one another and resume many endeavors that they have previously enjoyed. Activities offered may include physical fitness, meditation, table games, trivia programs, and patio activities. We also offer:

Social Services

Often times, the transitional period into a memory care facility can be challenging. Our social services team is here to assist families and residents during this time to make the transition as smooth as possible. We provide necessary information, manage requests and concerns, help care planning, and manage discharge planning for each resident. Our social services team can also help with arranging:
