Carina or Karina the nurse in the trach unit is the best nurse y’all have. Every nurse should emulate her. She is kind, compassionate and informative. She is caring and takes care of her patients well.

I am extremely disappointed with the care my grandmother has received at this rehabilitation facility. Since being admitted, she has experienced two falls, which is deeply concerning for a resident who should be closely monitored and cared for properly. On one visit, I found my grandmother sitting in urine and after having a bowel movement. When I notified a CNA, the staff member briefly looked and stated she was “dry,” despite the obvious odor and signs that she needed to be changed. I had to advocate for my grandmother and even assist with changing her myself to ensure she was properly cared for. As a family, we frequently receive calls stating she is “yelling,” but there seems to be little effort to understand her needs or address the underlying issue. The facility often smells unpleasant, staff members appear disengaged, and there seems to be a lack of urgency and attentiveness toward residents. Communication is also a major issue. Calling the facility is frustrating because phones often go unanswered, voicemails are not returned, and it is extremely difficult to get in touch with anyone for updates or concerns regarding a loved one. Families trust rehabilitation facilities to provide safe, clean, compassionate, and attentive care. Unfortunately, our experience has been very disappointing and concerning. I sincerely hope management addresses these issues to improve the quality of care for residents and their families.

What makes your place great is one LPN named Randy on the 3rd floor! She is approachable, listening skills are impeccable! My sister Gertrude who is 94 years old likes this nurse.

The worst facility by far! My grandmother didn’t make it to a room because I refused to leave her in their care. The 1st floor smelled of urine and the 2nd floor smelled like poop. Staff was no where to be found. The patients looked as if no one were helping them. There was no order and patients were all over the place. Elevator was broken. Staff seemed lost and had the attitude of not caring.
