Such a beautiful building! We love the location for our family to visit our mother. She has had a very positive experience there and loves the staff, food, and the way she has been treated. Scott has done an excellent job helping mom go through rehab and keeping her spirits up.

My husband Tom, had hip surgery in May and was transferred to Well Springs for rehab. This facility is top notch. The staff, doctors and Physical Therapist are well trained, friendly, supportive and knowledgeable. The Thank you ALL for a fantastic experience.

Our mother was admitted to Well Springs SNF following a hospital stay and was supposed to receive physical and occupational therapy to help her regain strength and independence. The evening she arrived, she was still able to walk. Unfortunately, the therapy she was supposed to receive—at minimum 3–4 times per week—rarely happened. Sessions were short, inconsistent, and often skipped entirely. Instead of helping her improve, staff frequently left her lying in bed or sitting in a wheelchair for most of the day. Over time, her muscles deteriorated significantly, and walking became nearly impossible. Eventually, she could no longer take steps without the assistance of a walking device. Basic care was severely neglected. She was not properly bathed or given adequate peri-care. Staff tried forcing her into Depends, despite the fact that she had never needed them in her life. As time went on, it became obvious this was simply because staff did not want to assist residents to the restroom. There were many times she would call for help and wait 30–45 minutes before anyone responded, often resulting in humiliating accidents. Her medications were mixed up on several occasions. We also witnessed feces- and urine-soaked bedding and clothing being left in her room and even thrown onto the floor. Our family met with management, including Heath, multiple times, but despite plenty of promises, nothing ever changed. We ultimately removed her from the facility, and within 12 hours she had to be hospitalized with a severe blood infection caused by unsanitary conditions and neglect, and bed sores on her back side. The night staff were by far the worst. My mother began recording the amount of time it took for someone to answer her call button because the delays were so excessive. She also suffered falls there—despite never having fallen before in her life. On multiple occasions, night staff left her lying on the floor with only her own pillow placed under her head while waiting for another employee to help lift her, sometimes for 20 minutes or longer. There were also several times our family arrived and found all the doors locked, forcing us to wait extended periods before anyone answered. Each visit, our mother appeared more neglected and distressed because of the poor care she was receiving. Late at night, children—presumably employees’ kids—would run through the halls loudly, disturbing residents. Her water containers were frequently left empty and dried out. This place was an absolute nightmare. We personally witnessed verbal abuse from staff toward other patients in common areas. Based on our experience, I strongly urge others to stay far away from this facility. Their negligence nearly cost our mother her life. SHAMEFUL!!!

Updated to reflect the comment on my review. My mother was there, but I didn't mention her name, so how would they know if she was a patient? Perhaps the attention to detail is not only lacking in the treatment of patients, but in their reading comprehension as well. Avoid at all costs. My mother was sent here to work on PT and OT after surgery. We were told she would have 1 hour of each per day. Unfortunately, someone would only show up every few days and work with her for around 10 minutes at a time. Upon entry, she was walking with a walker and bedridden when she left 3 weeks later, unable to even sit in a wheelchair. Although they were advised she cannot tolerate gluten, she was fed gluten every meal and would vomit shortly after eating. On at least 2 occasions, she was set in a chair while they changed her bedding and then left there, unable to reach the red alert button. After 45 minutes, she had to call my father, who called the front desk to go back to her. Due to covid, we were not allowed in the building. Had we known any of that was going on, she would have been gone immediately. The home healthcare workers who cared for her afterwards took pictures of her bruises and suggested she had been physically abused. I would never send anyone there and would give it 0 stars if possible!

My husband Tom stayed here for 4 weeks after his weeklong hospital stay for skilled nursing , OT and PT. I was able to visit Tom daily and what I witnessed was just second to none . The staff starting from admin personally visiting us in the hospital prior to admission explaining all details, the sweet welcoming receptionist to the case manager guiding us in D/C , the CNA’s, the LPN’s, the OT’s, PTA’s , the NP( love love her) the housekeeping and let me just give a huge shoutout to the dietary team for putting 26 lbs back on to my husband! The food was ALWAYS AMAZING for Tom , creative, tasty and nourishing . To the maintenance man who quickly fixed Toms heater and A/C. My husband was NOT an easy patient , demanding and sometimes rude , but he learned quickly that with patience you will get all your needs met as promptly as possible . The staff caring for him were incredibly concerned , patient , helpful, nurturing and kind. Always checking in on him , even playing games or putting puzzles together . It always felt like HOME …beautifully decorated, fastidiously clean , smelled nice and spacious rooms with bathrooms. Anything and everything you could ever need or want was always at your disposal. I cannot say enough GOOD about Wellsprings and should Tom ever have to revisit any rehab this would be the only center I’d allow him to go to. Thank you so much for making my husband well , keeping him safe and bringing him back to life. ☺️
