Worst place I have been to! The so called Nurse is rude and a poor excuse for a nurse! She is a disgrace to the nursing field! Made my mother on hospice miserable and feeling intimidated. Bed was stained with discoloration of urine! Left same day! I wouldn’t send my worst enemy there! Would NEVER recommend this place for your loved one.

I was there 3 months and care for the most part was subpar. Had a ankle fracture that NP never once looked at even after I had to go back to surgery because it got infected.Once i returned after surgery again NP. DON. And wound nurse never looked at. Discovered i was not getting right dose of Vitamin D and no one can tell me why that happened. Finally went to ER on Dec 30 and after iv antibiotics to help my ankle I ended up with a R below the knee amputation. As far as therapy one of the worst experience I've every seen and I used to be the night manager in Fort Wayne Ind Rehab Hospital

The therapists are excellent. I came here as a hoist patient, Toledo hospital saved my life, but Vancrest rehibilitation had me walking and able to eat and drink without choking and got the use of my arms again. The therapists are excellent and all the staff are so nice and very helpful and caring.

By far the best nursing I have ever seen. The staff were very professional, attentive, kind, and caring with my mother. It also did not smell like a nursing home! Very clean facility
