To Whom It May Concern, I am writing this letter to formally express my deep concern and dissatisfaction regarding the care my mother received at your rehabilitation facility. Based on our experience, I give this facility a rating of 0 out of 5 stars. From the moment my mother arrived, the level of care was unacceptable. Her bed was not functioning properly and had no safety rails, creating a serious fall risk. Her personal belongings repeatedly went missing, and staff showed little concern or accountability. My mother, who is vision impaired, was unable to sit up, eat properly, or take her prescribed medication for several days. Staff failed to provide a safe or effective alternative method for administering her medication despite suggestions being made. As a direct result of this neglect, her blood pressure rose to dangerous levels. After days of inaction, staff ultimately called 911 because they were unable to manage her condition—an emergency that could have been prevented with proper care. During her stay, my mother received physical therapy only once, which is completely unacceptable for a rehabilitation facility. By the second day, it was clear she was not receiving the care she needed, and I had to repeatedly question staff about transferring her elsewhere. The facility itself is unsanitary and poorly maintained. The building had a strong, unpleasant odor, and the environment felt unsafe. The food provided was inadequate, with no clear indication of what residents were being served. At night, the hallways were chaotic, with excessive noise and activity. Most concerning, residents were not being properly washed or changed. I personally witnessed this and had to come in daily to thoroughly clean my mother myself. No family should have to do this in a facility that is entrusted with the care of vulnerable individuals. While there may be one or two staff members who genuinely care, the overall standard of care reflects neglect rather than rehabilitation. Families trust facilities like yours with their loved ones’ lives, and that trust was severely broken in this case. Based on sanitation issues, lack of medical support, poor communication, and inadequate patient care, I cannot recommend this facility to anyone. I strongly urge management and oversight authorities to investigate these conditions to prevent other families from experiencing what we did. Please Do not send your loved ones here ever!!!!
