I need to revisit this issue, as my family member is still currently in the facility. The situation is concerning. The food provided appears and smells unappealing, similar to canned dog food. My family and I have been supplementing their meals by bringing additional food. Despite claiming they are not eating, it’s understandable given the poor quality of the food. Patients in wheelchairs are often left sitting around the nurse station or in a room with a broken television, sometimes unattended if staff members are distracted by their phones. My family member has not received a bath in over a week; the bed was changed by my family and me, and it had been at least six days. There are also issues with diaper changes, as they are not performed as regularly as needed, and sometimes patients are left in soiled diapers for days. Unfortunately, my family member has developed a pressure sore, which is new and likely caused by inadequate care. Overall, their condition has deteriorated since admission. It appears that without our providing food, their condition might be worse. The quality of care provided is very concerning. Additionally, I have observed staff members inappropriately taking items from patients. I have been approached multiple times by other patients asking if I have seen their belongings, as items go missing frequently. Although staff mention patient regression, many of these issues seem to be the result of neglect or improper care. Physical therapy does not seem to be provided consistently when scheduled, and when I speak with staff, there is a lack of transparency—some staff members avoid eye contact and appear untruthful. The facility administrator exhibits a dismissive attitude, and it seems staff do not take their authority seriously. His communication often feels dismissive or superficial, which erodes trust and respect. Furthermore, my family member has sustained injuries during their stay, which concerns us deeply. I plan to provide an update once my family member is safely transferred elsewhere. It is disheartening that honest reviews seem to be met with concern about potential neglect or abuse

Zero stars from us , even though the case manager at Mercy said Medicare lists it as a 5 star facility. My first clue that we'd made a mistake was when my feet STUCK to my mother's floor when I arrived after her admittance. The sticky mess had obviously been to here a while. Stolen items, no help with feeding, medicines not appropriately given, dirty, BS answers rudely given to our questions. Disgusting. Fortunately, we were able to get her transferred 3 days later. At that point, Mt mother stopped calling me to beg me to get her out.

The level of care is very good. They have a memory care area, they handle rehab and release cases. For full time residents there are activities, a good area to make lasting friendships. The staff is mostly fun people. If you have issues management is approachable.

If I could give this facility a 0 star rating I would. The social worker is rude as ever and no help at all. My mom’s case manager did absolutely nothing. The older Black lady with glasses at the front desk has a very smart mouth and she is rude as all get out. The aides neglect the residents. There is absolutely no communication at all between the staff. The day of my mother’s discharge nobody was aware that she was being discharged even though I had spoken to multiple staff members prior to her discharge. They could not locate her discharge paperwork when I got there. My mother’s luggage bag was stolen, and when I arrived she was the only resident in bed while all the other residents were in the sitting area watching television. My mother’s hair was matted to her head and none of her personal hygiene products had been used on her during her stay. Upon discharge the case manager was supposed to assist my family in setting up care for my mother at home as well as assist with getting her equipment that was needed. None of that was done. I’ve paid out of pocket and ordered her medical equipment myself. I had to file a grievance on an aide that was neglecting my mother when I popped up unannounced. The only staff member who was of any assistance was the Director of Nursing Erica. Please do not allow your loved ones to go to this facility. This place should be shutdown permanently. This is why my mother cane home. I will tend to her every need. She will never go to this facility again. I will be making phone calls to report everything that happened while my mother was there. If it wasn’t for her broken hip she would’ve never went. She has declined significantly in the 19 days she was there.

If you care about your family member you will not put them in this facility. My mother was there for about a month and she had one shower. I know this to be a fact because I would get there early in the morning and I wouldn't leave until 8 o'clock at night and there's nobody given showers that late she got one I believe on the 2nd day she was there and never had another 1 after that. A really sweet lady that worked as an aid there told me that they were not getting showered for weeks and she had offered to come in on her day off and get some of the patient showered and she was told no that they would not pay her to come in and do that. This is the kind of place that if your family member told you about this and you went to nursing or to the administrator they'd say oh well they're confused or they've forget and they're getting their showers every few days it's a lie. My mom was there because she had fallen broken her hip and had to have surgery at mercy hospital and this was the only place available for her to go. I didn't want to leave her at night but they wouldn't allow me to stay with her. My mom was afraid to stay there because she kept telling me a woman was coming in in the middle of the night and just standing over her bed and it scared her and when I went to the nearest station they all knew about it but said there was nothing they could do about it they couldn't restrain the patient or keep her out of my mom's room. I couldn't get any good rest when I went home because I laid in bed all night worrying about what was going on there. The final Straw was when my mom rang her bell and rang her bell and nobody came to get her to the bathroom during the night she woke up her roommate because she was yelling for over an hour for someone to come help her and nobody came to the room they didn't even come to make sure that she hadn't fallen out of bed. I was told by someone at the nurse station about this when I was heading to my mom's room and an investigation was going on. When I demanded to speak to the facility administrator he came down with a nursing supervisor and I told them what I was told by the person at the nurse's station and by her roommate and that because nobody came to help her she laid for hours in a wet bed. I told them that the woman at the nurse's station told me an investigation was going on and the nursing supervisor looked so shocked because she said well I would be the one investigating this and I haven't been informed of this situation. I told that nursing supervisor and that administrator that they have bigger problem than they probably even know about in that facility.
