This review is long over due. My mother broke her femur on Dec 22. After surgery and a hospital stay she was sent to The Pavilion at Creekwood. We didn’t know what to expect as this was a first experience for our family. The staff couldn’t have been more caring. Keeland and Lynette, 2 of the PTs, were especially noted for their positive and supportive attitudes. When Mom lacked confidence, they gave her theirs. Cynthia, her weekday room aid, paid extra attention to Mom’s needs and went overboard for her comfortability. Saudi, Mom’s evening nurse, was simply PHENOMENAL. She made a point of greeting her patients when she arrived and said goodbye when she left. She worked beyond regular hours making sure her patients were settled and comfortable before saying good night. Her room was cleaned twice daily in addition to being tidied if she needed it. I never worried about Mom when she was there because I knew she was in caring hands. Mom became friends with the staff and was sad to leave.

I would give this place 10 stars if I could but I guess 5 will do. From the time my mother stepped into this place they have addressed all of my concerns quickly and made sure my mother was very comfortable in her stay. From Kelli to Brenda to Mike to Tim to Will thank you all for making my mother feel like she matters and not just another check collected 🩷

Been here three times and it’s the best for price range. Food not bad either. Food is better here than the two other places I have been.

I would leave 0 stars if I could.<br>My mother and I entrusted this facility with my father’s rehabilitation after he fell, broke his arm, and injured his knee. He was transferred here on November 1 from Mansfield Methodist Medical Center, which had recommended the facility. From the beginning, we clearly communicated our biggest concern: my father was a high fall risk and needed close monitoring, ideally near the nurses’ station. Instead, he was placed in the last room at the end of the hallway, farthest from the nurses’ desk.<br><br>The condition of his room was concerning. The floors felt sticky, surfaces appeared dusty, and there was a musty smell coming from the air unit. Unfortunately, this was only the beginning of our concerns.<br><br>During his stay, we observed repeated issues with basic care and communication. My father was fully dependent due to mobility issues and dementia, yet we had concerns that he was not being changed regularly. His tray table was consistently placed on the side of his broken arm, forcing him to reach awkwardly for food and drinks. We repeatedly asked that his drinks be provided with lids, but this request was not followed, and we found spills during many of our visits. When we asked CNAs and nurses for updates about his care, we were often told, “I don’t know.” We also escalated our concerns to social workers and the director of physical therapy, who assured us the issues would be addressed, but we did not see meaningful improvement.<br><br>On November 11, Veterans Day, we received a call that my father had been rushed to the hospital due to dangerously high blood pressure and blood sugar. After spending the day in the ER, we learned he was severely dehydrated, required three bags of IV fluids, and had developed severe sepsis from a UTI. His health declined rapidly after that. He spent 14 days in the ICU, was transferred to hospice care, and passed away on November 28.<br><br>When we returned to collect his belongings on November 13, we found that the Director of Nursing/Business Office Manager had left a document and business card in his empty room regarding payments. No one had called to ask how he was doing after he had been rushed to the hospital.<br><br>Based on our family’s experience, I would strongly urge families to look elsewhere when seeking care for a loved one. We trusted this facility during a vulnerable time, and the lack of attention, communication, cleanliness, and basic care left us deeply disappointed and heartbroken.

My mother was here a few years ago. They’re very neglectful to people whose families aren’t there often. One of the staff cut my diabetic mom’s nails so short it caused her to get an infection which resulted in a finger amputation. They also called APS on my sister when she said she needed help when our mother returned home. She wanted case management and a PCA to help my mom instead they said she didn’t want our mom to return home, which was a complete lie.
