The Oaks

The Oaks is a skilled nursing and rehabilitation community in Winston-Salem, North Carolina, conveniently located across from Forsyth Medical Center. The facility provides a complete continuum of care including short-term rehabilitation, long-term living, skilled nursing care, hospice care, palliative care, and respite care.

Upon arrival, residents work with a clinical team to develop person-centered care plans designed to help regain skills and reduce hospital readmission. The community offers comprehensive rehabilitation services following accidents, surgery, stroke, or cardiac events through in-house physical, speech, and occupational therapists who develop individualized daily plans with achievable goals. Physical therapy addresses mobility, balance, coordination, and joint movement while managing pain, occupational therapy helps with daily activities like bathing and grooming, and speech therapy improves communication and swallowing abilities.

The Oaks features a well-equipped therapy gym with an Activities of Daily Living Suite, along with enhanced therapy services including diathermy, ultrasound, electrical stimulation, and vital stimulation to support recovery. Skilled nursing care includes wound care management, cardiopulmonary therapy, fall interventions, incontinence management, IV therapy, full-time physician coverage, geriatric psychiatry services, and podiatry services provided by registered nurses, licensed practical nurses, and certified nursing assistants available around the clock.
