I've been very impressed with the care and professionalism at Silas Creek Rehabilitation Center. The staff are compassionate, attentive, and truly dedicated to the well-being of their residents. The facility is clean, and the therapy and nursing teams work together to provide quality care. Thank you to everyone for the hard work and kindness you show every day.

I am disappointed with the level of communication at this skilled nursing facility. It is often difficult to get confirmation about transportation to get my loved one to his appointments in which they provide. We had to reschedule an appointment because the communication was lost among each of his nurses, clinical admin coordinator and transportation coordinator. Phone calls are not returned promptly, and it frequently feels like I have to repeatedly ask for basic information. Good communication is essential when families are trusting a facility to care for someone they love. This experience caused unnecessary stress and made it difficult to have confidence in the care being provided. I hope the facility improves its communication with residents' families by providing timely updates, returning calls, and ensuring staff are available to answer questions. Better communication would go a long way toward improving the overall experience.

I cannot overstate how much I disliked this facility. Patient care was aweful. My relative called the front desk several times for care and the response was the same. "I'll send someone". No one ever came. My relative had to call out to get some to help her. When medicine was brought there was no liquid to swallow it with. It had to be requested. It also had to be requested to not be given a cup with someone else's backwash. The staff at first handed her a cup that belonged to someone else. This may be a trickle down effect because the staff was only treated slightly better. The call button stopped working in the middle of the night. The response was to check the button note "well it isn't working. All I can do is call someone to come in the morning." No one had come at noon. Please take the time to read this review and ensure your family member has better care than mine recieved. I am terribly disappointed and I thank God for the few bright spots in that facility. Two nurses were very helpful and cared well for my member. I hope you two recieve blessings ten-fold.

My grandmother has been in here since the 19th of February. At first everything was great....until her bed kept messing up. For 4 days now it has not been fixed when the maintenance man said he fixed it. My grandmother's feet are terribly swollen. Everytime I come to see her I feel that she has been neglected. And when she pushes her call button I've noticed 1 time that it took at least an hour for someone to come see what was wrong. She's only here temporarily for physical therapy, maybe they do better with the permanent residents I hope. She won't come back here.

It was terrible the 4 weeks I was in. First shift was decent. Second shift staff would be laughing, cackling, and cutting up in the hallway when my nurse light has been on for over an hour to go pee. My wound care was not regular, nor did I receive the antibiotics or pain medication I needed on time. Third shift was the worst. Still laughing and cackling instead of working. The cna on third shift left me in my pee and clocked out. I called the front desk to report her, and after that she was always short with me. Laundry was not being done consistently. Attitudes were the only thing readily handed out. I'm glad for the few people who had kindness but this place is from hell.
