To: Patricia Mann, PT DPT<br>Director of Rehabilitation Services<br><br>I went to Green Fields Rehabilitation by choice after my knee replacement surgery. I was there for 13 days with unknown expectations. I could not have been more pleased going anywhere else. My Physical Therapists Molly Niezgoda and student intern Mo were wonderful to work with. Empathetic for the pain I was experiencing yet determined to get me strong enough and wise enough to continue on my own when released.<br>The staff of many nurses like Chris, Cindy, Amanda, Katelynn and Cheryl were professional and caring each time I encountered them. If I questioned a medication they told me and explained what it was. The aides were plentiful and mostly helpful. Maria, Brittany, Amanda and Charli proved to be dedicated, helpful and cheerful regardless the time of day. There were so many helpful aides I can’t remember all their names. There was one aide who seemed to be everywhere always encouraging interaction with patients and staff. Small woman with short hair and glasses.<br>I also had OT twice a day with Eribel Lopez who was easy going and pleasant to work with.<br>My stay at Green Fields Rehab was pleasantly surprising. Not just because the staff was good at their jobs. The atmosphere was congenial among all staff. I didn’t see or feel staff members loathing their job or disrespectful toward where they worked.<br>From my own past experience and current social values that is rare to me.<br>I highly recommend choosing Green Fields if you have the need for rehab.

Very dissatisfied with this facility. My mom is 95 years old and has dementia. We put her in the memory care facility two months ago. We had cared for mom at her home since 2020 when she had a stroke. A few weeks ago we were told mom would have to go to skilled nursing as they could not care for her. The issue was lack of bowel and bladder control. Staff are not trained to deal with this. Now this is one of the issues many people with dementia or Alzheimers have and most memory care facilities know how to deal with it. Mom wears depends all the time. She just needs staff to help her with toiletry, dressing etc. when I visited her the only thing I saw the aids doing was cooking meals. Staff also said it took two people to shower mom. When mom was home we only needed one. Total lack of training for the aids there. There were a few good ones. They wanted us to move mom up from a $8000 a month memory care to $18000 a month skilled nursing. I think they did not like mom or wanted her money. I would not recommend this facility to anyone.

I am very happy with the choice to send my husband to greenfields. Everyone has been very nice. PT is awesome. OT great. Speech fabulous and Sarah is very smart and extremely knowledgeable the reception staff always so polite and the aids were so helpful. The place was very clean and never smelt. So many others I saw were very depressing and smelly. Michelle the social worker is so sweet and thorough so many great nurses Kelly Kerry but I could list so many. They really look after you. Overall this is and had been a great experience. Love the place.

Please do not send any family member there for rehab. They will never work with them to get them out of bed or get them better. My mother came in for rehab and their lack of care resulted in bed sores and almost no mobility and leaving her wet in her bed and now wearing diapers. Please if you love your family member, find another place.
