Sundance is undergoing an impressive transformation. The staff are taking genuine pride in their work, which is evident in their attitude and level of care which is significant in a 5 star building. Leadership is strong, visible, and inspiring, creating a culture of accountability and excellence. It’s encouraging to see a community so focused on growth, teamwork, and delivering highER-quality service.

Where do I begin. First, my mil was deposited here after a short stay in another rehab. In the main level the rooms are 3 to a room. The room size was built to only accommodate 2 people, they squeezed in a third bed in between. It was dirty. The floors looked like they hadn't been washed in months or longer. There was a lingering fragrance of urine and feces. Everything was very old and unkept. To the staff. There were a few people that were nursing care that appeared to appreciate their jobs. A couple of CNA's did also but then there were the majority that were just there because they had to be. They needed a job, any job with a paycheck. The care they delivered was subpar at best. My mil has functioning dementia. She goes in and out of lucidity. We were told by neurology and psychology at the hospital that she could no longer make decisions for herself. In a nutshell, she was unable to care for herself including personal care, meals, and daily financial and other living needs. She had been living in very poor conditions and had been taken advantage of financially by family members, neighbors and home care staff including doctors and nursing. She had survived a fall in which she broke her neck. Social workers at the hospital were great they placed her at a very nice rehab facility. There was a problem with her MPOA and they could not get rid of her fast enough. Supposedly the only place in town that had an open bed was Sundance Skilled Nursing and Rehab. No one in her immediate family was able to care for her advancing needs. She is a smoker so options were limited. She was placed in the middle bed between two other woman. First thing we became aware of was her personal belongings went missing. She was given a tall dresser 5' plus with the tv placed on top of it. She had a neck brace that was ordered to stay on. It was taken off by staff and not put back on. She began hoarding food in the dresser and when the staff was made aware they claimed they were not allowed to go through her personal space. We cleaned out the dresser several times and request it be taken out of the room but it never happened. We removed her cherished belongings before they became someone else's. She was left with basic clothing. The next crisis came when another patient thought my mil had stolen her cell phone and physically assaulted her. The staff realized what had happened and removed the other patient and put my mil and the other patient under 24/7 guard. We were horrified! We began our own search for another facility. We soon found another placement and began the paperwork to move her which involved the office staff of Sundance. Once it was leaked out that my mil was moving the staff would say stuff to her like, "we love having you here, grandma" and "you know if you don't want to move you don't have to." By then we had secured a MPOA and FPOA and were very much in charge. She was moving. The staff continued to undermine our attempts by stalling paperwork, making her afraid to move to a new place using fear tactics. Even telling her they were going to miss her so much as she was part of their family. We threatened to turn them in and they did back off once litigation both individual and facility wide was threatened. We were taking names and threatening licenses. Also at this point my mil began to smell. We asked when she had been bathed last. The staff said she had not had a bath at all. She had been refusing a bath all along. So no bathing was done. The staff claimed that they can't make her bathe. Her linens were not changed in the month she was there ei It was basically a frat house for the disabled and elderly. The day before we were moving her we went to pack her things. It was Halloween. As with many nursing homes, children were visiting to trick or treat. No one noticed that when the children left on their way down west Colorado Avenue my mil left with them. We arrived and she was gone. No one knew when she left nor how long she had been gone. We panicked and drove around looking for her. Six blocks away we found her. She was scared and disoriented. Not anyone took responsibility from Sundance. We took her and left. If I could have given a negative star rating I would have.

The Activity program at Sundance is amazing! Tabatha works hard to see that every resident is involved ~ she is not just meeting their needs, she is going above and beyond!

The CNAs, Nurses, and dietary aides work very hard to care for the residents here. You can tell they want people to be rehabilitated as fast as possible.
