My dad was transferred to this facility after spending a week in the hospital recovering from a hip replacement and a broken femur. He was transported directly from the hospital and placed in a corner room that was extremely hot (at least 80 degrees), with paint chipping off the walls and a strong odor that suggested poor hygiene throughout the facility. There were even spiders on the floor. The most concerning issue was his pain management. The nursing staff either did not have or had lost his prescribed pain medication. Because it was the weekend, they told us there was nothing they could do. They actually allowed my mother to go home, retrieve two-year-old pain medication, and give it to him while he was a resident in their care. Several people who had previously worked at the facility told us they would not send their worst enemy there. After seeing the conditions firsthand and realizing my father was not receiving appropriate care, we called an ambulance and had him transported back to the hospital so he could be placed somewhere else.

My mother, a dementia patient who had broken her hip, recently spent about a month rehabbing at Sullivan Park Care Center (skilled nursing facility). The staff at SP were wonderful! They were always upbeat and positive with mom, and kept a close eye on her. They also kept me, her caregiver, regularly updated and did a great job of coordinating her rehab and care with me. Her rehab went great and she is doing very well. Now that mom is back at her memory care facility, I am very glad that she was able to rehab at Sullivan Park. Thank you to all the staff there!

This place is HORRIBLE! Our grandmother was there not even 48 hrs before we called the ambulance to get her out of there. The staff does not communicate with each other or know what is going on with new intakes. Our grandmother was very ill and had been in the hospital for a week prior to coming here. When she left the hospital she was talking and STABLE- after the time she spent here it was like she went backwards. They did not assist her with basic needs that were clearly stated on discharge paperwork. When we tried to ask questions about what went wrong the facility manager didn’t give any sympathy to our family or ask how my grandma was. She was practically dead when she went back to the hospital. Then the next day the office lady asked if we wanted to pay to hold her room for her to return. Are you joking?!?! All they care about is $$$$. Do not send your loved ones here! Period.

I would suggest anyone who need physical help come here. This is a blessed place. It has helped me get stronger without force or putting pressure on me. The food is good. The people are great. They help teach you how to take care of yourself. I hope whoever is reading this will come here. Thank you all for your time and patience. God bless Sullivan Park and the people who work here.

I can’t say enough good and positive things about Sullivan Park. All the way from check-in till discharge i was treated with the utmost respect and professionalism. Most importantly, they truly care! But, I especially want to thank all the nurses, physical therapy , associates and all my peeps in 100 Hall. I spent just under 30 days and the staff always came in every single day with a smile on their face willing to do whatever to make sure everyone was taking care of!
