Oh my goodness I would definitely recommend this place the staff here is kind they patience they fare I want to give a special shout out to Miss Palmer she gets things done accordingly to your needs also I want to give a shout out to Angelita she's a social worker here she'll help you get through whatever you need to get through willingly also they have a concierge lady her name is Jessica she'll get you anything you need pertaining to you transportation your medications anything pertaining to your care she'll make it happen she'll work with you also I want to give a special special shout out to Brianna she's the she's the recreation lady here this girl is do your nails she'll do your hair oh she's really fun to be around with that said I want to give a shout out to all the nurses Also just to let you guys know that they have a day care recreation center they still come here but it's still part of split Rock even if they're not patient at split Rock I would recommend it with that said just want to thank you guys again for your time and patience and y'all continue to be blessed because you are Sincerely Ms Brown 🤪🤗🐞

I just wanna shout out Split Rock for being apart of my healing process. Esp. Patel (Physical therapist) Millie (Occupational therapist) The nurses Diana , Jamie , Ducket for being so supportive through our talks laughs and just being understanding. Ms. Levi (CNA) who goes above and beyond for all the residents and forget about herself often. Ms. Harris deserves a major shout out. And to my fellow residents Harold Anibal Franko O Dawg. Please trust the process and trust the team who is behind you and believe in yourself. Thank you again everyone for the help and support as needed and wanted.

DO NOT BRING YOUR LOVE ONES HERE!!!!!!!!!!!!! My grandmother was transferred from Henry J Carter to Split Rock. She was weaned off the ventilator and she was doing phenomena UNTILL SHE GOT HERE!!!!!! I don’t know how this place has soo many good review because the nurses and staff here are TERRIBLE!! Lady (the supervisor for the nurses is the worst and the other nurses under her are even worse. Respiratory team sucks as well. They leave the patients full of mucus hours on end while they sit in a small computer room looking at their phones pretending to be typing. I had to speak to their super visor (Hank) because RT didn’t want to suction my grandmother and was telling her coworker with an attitude that we needed to wait. So I went straight to Hank and they suctioned her. Then a couple days later my grandmother complained about the same respiratory therapist because she had an attitude and was grabbing her too roughly. So I addressed it to Hank and to AVI (The director of operations) and nothing got done. I also reached out to him about my grandmother’s health and that something didn’t seem right with her and that it looked like her health was deteriorating. The director of operations said he would call me and never did after expressing my concerns about my grandmothers health and other complaints: Then that same day I left split I get a call stating that they let my grandmother FALL!!! My grandmother is 87 years old! The bed they have her on has no rails!! We requested the rails the first day she got there and they never got them and then they let her fall. Unacceptable!!!!!!! My uncle visits my grandmother at night. He found her drenched in her own urine. So he asked a nurse to see if they can bring some one to change her and NO ONE SHOWD UP FOR 2 hours!!! So imagine how MANY HOURS she was left there in her own filth!!!!! No imagine all the other patients in this place who are neglected and mistreated everyday and night!!! And….. TODAY…after a wonderful morning with my grandmother. I fed her we spoke for a few. She was responsive but a little tired. They were about to change her and I usually leave around 2 but I waited till they came at 2:15-2:20pm and left. An hour later I get a call from from Lady (nurses supervisor) that my grandmother wasn’t responding and that they were going to perform cpr and that the ambulance was on the way but didn’t wan to give me any information stating if she was alive or had a pulse. She kept dodging the question untill she finally said “yes, she had a pulse” but that she would need to call me back in 10 min to update me. Imagine you finding out your family member was unresponsive and the HEAD SUPERVISING NURSE AT THE FACILITY DOES NOT KNOW WHATS HAPPENING!!!! Too many incidents happening at this place and mind you she’s rarely left alone by any of us. We take turns coming to see her in the morning and in the afternoon for these same reasons and these people still find ways to hurt my grandmother. Yes, I said hurt because it’s now becoming a pattern. A pattern the state and an attorney will soon investigate. PLEASE DO NOT BRING YOUR FAMILY MEMBERS HERE BECASE IF YOU MAKE ANY COMPLAINTS OR START TO DIG AROUND THEY WILL NEGLECT AND DO THINGS TO HURT YOUR LOVEONE.

Split Rock nursing home is truly exceptional. The staff is compassionate, tentative and treats every resident with the dignity and respect. the facilities clean comfortable and creates a warm welcoming at environment. I feel confident knowing my grandmother is in great hands. I highly recommend for anyone seeking quality long-term care especialy those in need of a ventilator

We chose this facility based on their online reviews but from having a loved one here for the last two months if I could give this place zero stars I would. Since my family member has been placed at this facility her health has drastically declined. She was doing great when she was transferred in. The goal of placing her at this facility was to move to the next phase, physical therapy so she could come home and that has not happened. This place is seriously outdated. Everything is in drastic need of an upgrade. The place is not clean and looks dingy and feels dirty. I visit after work and am disgusted by the night crew. They do not care about their patients. If you do not visit daily your family member is at huge risk of being severely neglected. They have had two people die in the last two weeks. They leave the patients sit in their own filth FOR HOURS. If you ask for a CNA to change them you are given an attitude or ignored. The nurse Jennifer is highly incompetent and is a serious risk to her patients. I have seen her try to give meds and the meds start squirting everywhere. Then wants to leave my mom wet and nonchalantly say they will be by later to change her. They are aggressive and rough when they change a patient because thankfully my loved one can talk. There is a serious issue of neglect in this facility. There is a man who is part of respiratory and he is also very rude and nasty. It seems like the entire night crew hates their jobs and it shows. There are maybe two or three staff who are courteous and offer their help. Other than that it's horrible. We are trying desperately to get her out and are highly concerned for her safety. She has even told us I'm going to die in here if you don't get me out of here. We are at our wits end and our hands are tied until another bed opens up at another facility. DO NOT leave anyone you love in this hell hole!
