Most of the nursing staff were very friendly, caring, & attentive but there were a couple who were hateful, would tell lies about residents, and clearly violatef confidentiality laws by standing in the halls talking to the DON about residents who could overhear them while being in the Family Room. I f one resident could hear discussions about residents between a nurse & the Director of Nursing/Charge Nurse, other residents & visitors to the facility could hear the same discussions about residents, which is a clear HIPPA violation. Any & All discussions about residents should be behind closed doors at ALL times! While I was a resident there, I was passing through the dining room on my way back to my room and witnessed another wheelchair bound resident in the floor where he had fallen out of his wheelchair. I went to the nurses station for my hallway and reporting that he had fallen out of his wheelchair and requested that the nurse call to the nurses station for the hall that this gentleman's room was on to get the RN or CNAs to go get him out of the floor and put him back in his wheelchair. Then an hour later, I was passing through the dining room once more and he was STILL laying there in the floor with his wheelchair & hat moved to the other side of the room! There was also a different resident who is paralyzed from the waste down & required CNAs to get him up out of the bed and into his wheelchair every day (or even the LPN/RNs could have done it) and there were many days than he would call down to the nurses station to tell them that he was ready to be gotten up (usually between 6 & 8 am) and I would go to his room to visit him a couple of times a day, sometimes as late as 9 pm & no one had ever came to his room to get him out of bed and into his wheelchair. So this guy was forced to lay in that bed for close to 20 hours at a time, sometimes longer than that after going to bed for the night, no one helping him out of bed, then having to spend the entire night laying there again until they came in to bring breakfast to him the following morning. While the facility was nice and most of the staff were caring & took care of the residents' needs, there were a few staff members that at the very least needed to reprimanded.

Southgate is the place to be for all of your healthcare needs. They have a beautiful rehabilitation wing for short term therapy stays, as well as, long term care needs. Caring, compassionate management and staff.

If I could give it less than a star I would. Things are done here that shouldn’t be done to anyone, and things aren’t done that are needed to be done. If you try to complain to Beverly who is head of … quality of life ( what a joke ), she gets defensive and says nothing wrong is happening, rather than say I’ll look into it,I’ll take care of it ,it won’t happen ever again. They have a few staff members who are professional.Then there are people that couldn’t care less about your loved one. They will leave them in pain . They won’t clean them. If your loved one is unable to feed themselves, they will likely starve . I’m going to seek legal advice . Don’t send ANYONE to this place !

My mother was there for short stay rehab. She received great care! I couldn’t visit due to covid but the staff were very helpful in making sure we could talk via phone and visit with FaceTime. Love this facility!!

Very disrespectful management. That nursing home has become more of a separation from rich and poor instead of worrying about the residents. Very very disappointed
