I visited this facility on April 26, 2026 and area where the clients live smells like urine. It was so bad I had to leave my family members room. This is unacceptable. Also ADA requirements are low. You do not have automatic access doors to the client rooms. I observed clients in wheelchairs unable to open the double doors because their wheelchair blocked the doors from opening. There should be a push button pad on the wall for the wheelchair clients to push in order to gain access to the common area on the first floor. My family member is not being treated fairly. On numerous occasions she has complain about 3 nurses however as a retaliation of her complaints she is being discharged in about 3 weeks. One nurse refused to identify herself to my family member nor me when she refused to give my family member her medication. This is also unacceptable. I know I won't get a response.

My Mother was here and had excellent care! The OT was outstanding. Everyone truly cared and yiu coukd tell theyvreally gave their heart and soul to their job. Thank you for the great care each of you provided for my Mother and for the concern about our Mothers needs

I enjoyed my visit today. The people were very pleasant including the residence that walked by. I would like to say that l came to hv lunch with my pastor, but after entering inside there was a strong urine odor in the hallways. The smell was so strong that l wasn't able to hv lunch with my pastor, but my visit was great. Other than that everything was awesome!

Zero stars for this facility. The moment you walk in, the smell is overwhelming — like a urine-soaked, moldy basement. The building is extremely understaffed, and there is no welcoming presence at the front. It honestly feels as though anyone could wander into a resident’s room unnoticed. I came to visit a loved one and was deeply upset by the poor upkeep and lack of attentive care. During my visit, I witnessed a patient call for assistance and wait 53 minutes without anyone responding. When a nurse finally entered the room, she rolled her eyes, walked out, and did not return for another 29 minutes. That level of disregard is unacceptable. Healthcare facilities should be places of compassion, urgency, and professionalism. Residents deserve positive, attentive caregivers — not staff who appear unhappy or unwilling to do their jobs. If you don’t want to care for vulnerable people, this is not the right profession. I sincerely hope serious improvements are made for the sake of the residents and their families.

The second floor staff has been extremely disappointing. They are clearly understaffed, and it shows in the level of care patients are receiving. Phones often go unanswered, and the call button for assistance is frequently ignored. There is very little empathy shown toward patients who are in pain or simply need basic help. I have a friend who has been there for almost three weeks and has only received one shower the entire time. When she needs assistance, she often has to ask me or another relative to call the nurses’ desk because no one responds to her calls for help. That should never be necessary for someone who is supposed to be under professional care. It’s deeply concerning that people who cannot care for themselves are being left waiting and feeling neglected. If staffing is the issue, then more staff need to be hired. Patients deserve timely responses, compassion, and basic dignity. Unfortunately, that has not been the experience on this floor.
