It's so disappointing to see such a vibrant & upbeat memory care facility to go straight to the pits. We've been on board a couple of years. The sell from Beth Shalom to the Gardens was very recent at the time.The upkeep was still excellent. The grounds very nice with clean accommodations. They had a very active schedule and looked after the residents overall well being with permanent employees. Oh My! Everything I described above no longer exists. The facilities upkeep, staff, food, activities & overall care is just sad. The residents wander aimlessly. All of the furniture and interior is unrecognizable. I have to now bring a seat with me to visit. All of the original management and mist of the staff are gone. Replaced by a temporary agency. The trust and repor with the residents and families is gone. Temporary staff do not have anything invested and it shows. The photo shows the 4th of July this year. What a difference a year makes! The residences, the last couple of years, enjoyed a garden they participated in planting and growing, cookouts, ice cream socials are now forgotten. I cannot express my dissatisfaction and disappointment. God bless the remaining, as very few are left.

Safety and Communication Failures – Management Please Respond Update on Review below: The facility replied asking me to call, I called and left a message for the Administrator 2 days ago and have not received a call back, again. Also still no response to my email from 9 days ago related to concerns on wait time for assistance. While the Physical Therapy team has been professional and helpful, the nursing and administrative failures at this facility are a serious concern. I am posting this publicly because my repeated private attempts to contact leadership have been ignored for over a week. 1. Patient Safety & Neglect: My family member, who is a fall risk and requires assistance for transfers and toileting, consistently waits 1–2 hours for a response to the call button. Staff frequently turn off the alert and promise to return, only to leave the patient waiting for hours. This is a significant safety and dignity issue. 2. Complete Administrative Breakdown: I have left numerous messages for the Unit Manager and Administration that remain unreturned. Even simple coordination, like confirming transport for medical appointments, is ignored. 3. Unprofessional Oversight: It is telling that the Administrator’s voicemail greeting is still set to a different person’s name. This lack of attention to detail reflects the lack of oversight on the floor. I am still waiting for a return call regarding my family member's care and transport schedule. If management is reading this, please check the multiple emails and voicemails sent by Ms. Moneymaker over the past week.

I have a family member at SG and have communicated with the new activities director. It takes a lot of work, energy, creativity, patience, compassion, and imagination to plan and carryout all the recreational activities for the residents. She checks “all those boxes” with the assistance of the other activities staff. She talks to the residents about their interests and their concerns. It’s impossible to make everyone happy, but she would like to. She is friendly, caring, and concerned about the well being of the residents. There have been parties, lunch outings, and shopping outings. It has been a pleasure, and I look forward to seeing what else she has planned.

I had an awesome experience with the services that were provided to my family member at shalom Gardens, Health And Rehab rehabilitation center. The staff was very knowledgeable had great customer service and were very professional. The Administrator went above and beyond just with the accessibility, the family like welcoming atmosphere And made you feel as if your family member was one of his own. I really appreciate it all the efforts that were put forth and the great care that was given. I would recommend this facility to anyone in need of rehab services.

Shalom Gardens has the most incredible rehab services! This talented team brought my Mom back from a very tough hospitalization to her pre-hospital baseline—maybe better. When she arrived at the facility she was only 80 pounds, unable to walk, speak plainly or perform any tasks of daily living. She is leaving 30 days later at 94 pounds, walking, dressing, talking clearly, laughing, playing bingo and planning a night out for pizza! Thank you SG rehab team and care staff for your compassion and amazing care!
