Senior LIFE York

Senior LIFE York is a care program in York, Pennsylvania, serving adults age 55 and older who qualify for Medicaid and meet the state's level of care requirements. For those who qualify and live in the service area, there is no cost to join the program.

Senior LIFE provides comprehensive services designed to help participants live safely and independently at home for as long as possible. The program offers rehabilitation and a variety of other healthcare services available 24 hours a day, 365 days a year through an interdisciplinary team approach.

The program's healthcare professionals specialize in geriatric care and include doctors, nurses, specialists, physical therapists, home care aides, and others. These professionals are dedicated to providing individualized care based on each participant's specific needs. The LIFE Health and Wellness Center operates Monday through Friday from 8 am to 4:30 pm.

To qualify for Senior LIFE, individuals must be age 55 or older, meet the level of care determined by the state, reside within the Senior LIFE coverage area in York, and be able to safely live within the community. In-network and out-of-network emergency services are covered by Senior LIFE, and participants may disenroll from the voluntary program at any time.
