About Senior CommUnity Care of Northern Kentucky PACE

Senior CommUnity Care of Northern Kentucky PACE is a Program of All-Inclusive Care for the Elderly (PACE) serving seniors in eight counties in Northern Kentucky, including Florence. Developed through partnership with Volunteers of America National Services, St. Elizabeth Healthcare, and Baptist Life Communities, the program provides comprehensive health care to older adults who would otherwise require nursing home care but can live safely in their communities with appropriate support.

The program coordinates care through an interdisciplinary team and covers a full spectrum of medical and social services tailored to each participant's needs. Services include primary and specialty medical care, prescription and over-the-counter medications, adult day services, home care services, mental health and social services, skilled therapies when needed, and nursing home and hospital care when necessary. Participants must be age 55 or older and certified by the state to need nursing home care, though more than 90 percent of PACE participants are able to continue living in the community despite significant care needs.

The program provides transportation to and from the PACE Center and to medical appointments based on individual needs. Eligible participants may qualify for Medicaid assistance with monthly premiums, with Medicare covering additional costs, though those not Medicaid-eligible can enroll by covering the Medicaid portion themselves. All necessary prescription and over-the-counter medications determined by the care team are covered, and the program offers family caregiver support including training, support groups, and respite care to help keep loved ones in the community.

Participants are free to dis-enroll from the program at any time and resume their benefits through traditional Medicare and Medicaid programs. Senior CommUnity Care of Northern Kentucky maintains a PACE Center where people can inquire about eligibility and services, and staff can help determine individual Medicaid eligibility and program fit.

This program is amazing. My mom started in February. I was beyond burnt out. Being my mom's whole social world was mentally exhausting and stressful - not to mention tasks like cooking, cleaning, helping with ADLs, transporting her to doctors, etc. To top it off, SHE was miserable and lonely. I was bending over backwards to help her but it was simply too much for me and not enough for her. I found this program by accident and when I heard about it I thought it sounded perfect. My mom is disabled from a stroke but she is only 57 and is very social and needs stimulation. PACE has given her something to look forward to every week. Something to get up for in the morning. And that's just the day center part, which is one small portion of what the program offers. She has received much better medical care than she has since the stroke happened, with regular PT, OT and speech therapy with brilliant therapists. She receives weekly meal deliveries, pre-sorted medications, a free fall alert pendant, help at home, and transportation to and from the center and appointments. She is making massive recovery strides even though it's two years after the stroke happened - which people tell you won't happen, but that's probably mostly because insurance doesn't cover it. That's another thing: we don't have to fight with insurance anymore. In 2.5 months she's received dozens of hours of therapy, incredible attention to her adjusting her medications and other medical needs, and several exams and tests she was years behind on. This program has been such a lifesaver for both of us. The kindness, attention, care and thoughtfulness put into the services offered are actually incredible. It's an all-inclusive program made to fit each individual participant - if the participant needs help with something, they probably already have a solution for it. My mom was in assisted living for 6 months and this is honestly better. Any time she needs something, PACE tries to take care of it. The staff are all so sweet and down to earth. I'm really blown away and hope the program lasts forever and more areas adopt it because it may be the best available solution for the aging population. It's so wonderful to just be a daughter again.
