Senior CommUnity Care of Colorado PACE - Montrose

Senior CommUnity Care of Colorado PACE in Montrose is a Program of All-Inclusive Care for the Elderly (PACE), a comprehensive healthcare program serving individuals over 55 who wish to remain living independently in their own homes and community. As the first PACE provider to operate in Western Colorado, Senior CommUnity Care offers coordinated healthcare, medication management, transportation, in-home assistance, and specialty care options to eligible participants.

The program is designed for frail, older adults who are able to live safely at home and in the community with appropriate support at the time of enrollment. Senior CommUnity Care PACE provides all covered services at no premium to participants who are eligible for both Medicare and Medicaid, or for those eligible for Medicaid alone. All prescription and non-prescription drugs, along with office visits approved by the interdisciplinary team, are covered 100 percent with no out-of-pocket costs for approved services.

The Montrose program is led by Center Director Trisha Martinez, a nurse professional with extensive experience in the organization. Senior CommUnity Care emphasizes preventive care and collaboration with participants' care teams through an interdisciplinary approach. Participants can retain their primary care physician, as the program contracts with many community physicians in Montrose and Delta counties, or utilize the PACE physician if preferred.

The program ensures access to care 24 hours a day, 7 days a week, 365 days a year, with registered nurses, physicians, and center administrators available by phone for any health concerns that arise at night or on weekends. Participants visit the PACE center on weekdays for services and support.

Enrollment is voluntary and flexible; participants are free to disenroll from PACE at any time and resume benefits in other Medicare and Medicaid programs, with disenrollment effective on the first day of the month following the request.
