Seasons Memory Care at Stone Oak is a small-community memory care facility in San Antonio, Texas, specializing in Alzheimer's disease and dementia care for approximately sixteen residents. The community provides 24-hour professional support with an emphasis on creating a home-like environment that prioritizes dignity, safety, and personal attention.

The community is built around a caregiver-to-resident ratio of approximately one caregiver for every four to five residents, allowing staff to know each person closely and respond more personally. Care includes assistance with everyday personal needs such as grooming, bathing, and meals, provided by licensed, bonded caregivers. The environment is designed with safety features to prevent wandering while supporting residents' freedom and independence. Seasons supports residents with complex dementia-related behaviors, including agitation, resistance to care, wandering, sleep disruption, and anxiety, with each prospective resident individually assessed before admission.

The community offers home-cooked meals prepared to meet nutritional needs, daily activities and engagement designed for cognitive challenges, medication management with physician and pharmacy coordination, and access to a visiting geriatric psychiatrist where clinically appropriate. Founded by Mona Talukdar, a licensed Physical Therapist, the community maintains a hands-on leadership approach, with the founder remaining actively involved in daily operations. Seasons operates as an all-inclusive monthly rent model without automatic annual rent increases, with pricing based on assessed care needs and any separately agreed optional services.
