Please please please don’t send someone that you love to this place!! My husband is at this ‘facility’ and has been there for 12 days now….he was sent there after a stay in the hospital & was sent there for ‘skilled nursing and Drs.’ He has seen the doctor ONE TIME in 12 days!!! The Doctor told him that he was going to take out his catheter and never came back, so is still in. Also, my husband has a lot of chronic pain from problems in his shoulder (he needs his shoulder rebuilt)….one day this terrible place actually ran out of pain pills???? He didn’t get one until 4:30 pm!! I’m horrified and all this place seems to do is barely do the essential work!! It’s dirty and I’m sure there’s good things about this place, but I certainly don’t know of one!! They have 3 people to each room!! One of the patients in my husband’s room yells a lot so is kinda impossible for my husband to get good sleep or rest. There’s no chairs for visitors to sit and visit with the patient in their room but do have big rooms to take the patient by wheelchair to so at least a person can go there to visit. I’ve never had experience with these so called “skilled nursing facilities”, so have nothing to compare it to be fair. But I simply think that it’s gross and incompetent to be so understaffed to not be seen by a Dr at LEAST once a week and to run out of pain pills is absolutely terrible for patients that are suffering in pain!! This place is very understaffed!! I will say that I’ve talked with a few nurses and they are understanding but they need more help!! He was sent there also for physical therapy…nope…has had ONE visit from the physical therapist for maybe 10 minutes.

I'll keep this short, even though I have a lot to say. This is a sorry excuse for a facility! They are understaffed and have poor care standards. Dignity is at the bottom of the priority list for them and that's absolutely shameful.

If you are having a hard time making a decision between rehab facilities and you need Physical and/or Occupational Therapy The Therapy Department at Meadow Park is run exceptionally well. Personally, I have had inpatient, outpatient and in home PT and OT and the people in this department are all the best I have ever worked with. I credit each of them for preparing me to go home and not be a burden on my family. As far as the rest of the facility it has all the usual issues you would expect in what is essentially a nursing home. There are nursing staff that are worth their weight in gold there are others that need a career change. The staffing is frustrating from a patient point of view. It just feels like my insurance is paying a fairly large amount of money each day that I should not have to skip a shower or wait to get help to the bathroom because of staffing.

Currently I have a family member living here for rehab from many months in OHSU ICU after a heart attack/stroke and is loving it. She's happy and getting herself back to health with the help of the wonderful staff she raves about and introduces us to. The physical therapy staff is amazing here! I have sat in on a session and saw it for myself! The place needs an update for sure but when you're family member is receiving this kind of love from staff and is this happy and progressing, who cares as long as everything is functioning and she has all she needs that's all that matters.

This is a Hell Hole ! This place stinks of feces & urine. dim lighting, shared room's. My Dad's room mate is a BULLY the nurse staff is down right mean. My father called me to tell me the nurse didn't give him his night time med & she said she did. He is in pain but they only give him Tylenol ! He's had one shower in over a week, the therapy he was supposed to do is not happening so he is not getting better & will be evicted on Veterans day due to state insurance cause all this place cares about is money & good insurance. I'll have to snap pic's next time I'm there but you have to visit for the smell.
