This place is bad they take for ever for my mom to be seen by a specialist took me to be on them after months for her to be seen she told me the food is always the same mostly sandwiches the cna taking care of my mom is amazing but whoever is running this place is rude I heard them yelling at the cnas when I walked by they don’t appreciate their workers but if it wssnt for the cnas I would’ve taken my mom some place else sooner the cnas have been great they don’t deserve to be treated that way and I can never get through to ask how my mom is doing I’m will be taking her out soon

DO NOT SEND YOUR LOVED ONES HERE. No rehab facility is going to be great, but this one was grossly negligent. My mom and I arrived to find that my dad had not been changed, hadn’t been eating/drinking but wasn’t hooked to an IV, and was crying in pain. I had to request that my dad be cleaned; during that process, he was screaming in pain while the nurse was arguing with him saying, “you’re fine, stop that.” As it turns out, he had begun the process of SEPTIC SHOCK, acquired over time at this facility. When the doctor finally came around and we discussed his symptoms, the doctor literally said, “Oh! He should have been transferred weeks ago!” No kidding??! Where had you been this entire time?! Eventually, an ambulance was called, but by then it was too late. After a lot of unnecessary pain, my dad passed away. Again, do not send anyone you care about here. None are great, that’s true, but this one is by far the worst.

Peter is the worse charge nurse there. Doesn't care about his pts, ALWAYS on the phone talking to his gf in his language. my dad fell multiple times and i was never was noticed about it. Never got a call from this horrible facility. now my dad is in critical condition in the hospital. Peter should have gotten his license revoked but i guess CDHP and rialto post acute doesn't care.

This facility is what you see is what you going to get. I have several times gone over with staff in regards to my husband's medication, his appearance, and complaints of the CNA disappearing. The therapy is great but the nursing is horrible. Being a Kaiser patient you would think you would be placed in a better environment for rehabilitation. Good thing this is only temporary and not long-term. This shows me that Kaiser cares less about the patient and it's all about a dollar. My suggestion to anyone that loved one is here and you're having problems, contact the ombudsman office and file a complaint and hopefully changes will occur. A little update today 08/20, I strongly recommend that you do not send any of your lungs to this place. I was questioning the administrator Juanita about my husband's therapy when she was not getting. I question about the care again that he was not receiving. When you question them about the facility they have a tendency of calling the police on you. I had to call California Department of Health on on them 3 times. And also had to report some CNAs because of the care they were providing my husband. Not to mention, they refused to let my husband come home and forcing him into a long-term facility when he has a home to come to and family to care for him. I beg you people do not send your family to this facility, you will regret it.

Overall I give this place a good review. It’s clean the food is edible and the skilled personal are very professional. I would like to thank Nurse Stacy Willam’s for always going above and beyond to take care of her patients. I can tell she cares about people and I appreciate that. She made my stay here that much better and my recovery a lot smoother.
