I have seen a tremendous improvement in the quality of care given at this facility the past few months. While they still have a ways to go, they are most certainly moving in the right direction. I am confident in the care my loved one is getting.

I'm currently a resident at Perry Creek.. I am mostly pleased with the staff, but some go above and beyond. The kitchen supervisor Miss Tracie Godley really cares about her job, and if there is any problems she will always make it right. Thank you Miss Tracie ❤️<br><br>It is now 8 months since I posted the above review. I would like to add that Miss Tracie still does a fantastic job, and she never lets me down. She is very committed, and the residents on my hall all love her as well. It isn't easy being a resident in a nursing home, but having someone like Miss Tracie around , who truly cares, makes the days alot more pleasant. Thank you again Miss Tracie. ❤️

I recently was a patient at Perry Creek & Rehab and received exceptional care and service! The facility provided transportation from the hospital to the center. They intake was so organized! I arrived at dinner time andy meal came shortly after I got settled into my room. I was given communication as to what to expect the next day.<br><br>The meals were good and plentiful. The staff, all of them. Were professional, kind, caring. The OT And PT were fabulous. The nurses and assistants responded to my needs expediently.<br><br>The discharge went smoothly. The nurse reviewed my stay and the follow up visits with primary care physician were scheduled prior to my discharge. It was nice to have the best service.<br><br>I do highly recommend Perry Creek Health and Rehab

This is a well maintained and great facility. My grandma Lottie has been in their care for years, and the staff has been friendly and attentive in their care with her. Tabitha in the Medical Records/Transportation department has been so helpful and has played a key role in the easy and efficient transition here for us.
