Rehabilitation facility: Very poor communication. Uneven care - occasionally responsive, but very inconsistent. Discharge planning totally inadequate. Overall grade: poor.

Call lights are for patients are on from 10 to 35 minutes. Food is served cold. Information is not documented and given to doctors or on site nurses. Invisable doctor Schieder (in 2 montgs I never met him)Needy patients are given so much attention that the rest if the patients are ignored. Simple things like brushing of teeth, washing hands prior to a meal. information is not properly related between shifts. Even after 2 weeks of follow up or an attempt to follow up not much has changed. my mother isn't sleeping still. A lap blanket (orange with blue crochet edge is missing, or stolen) which is a huge comfort for my mother in 309 disappeared at PT. Michael nurse practitioner just kept throwing more meds and now my mom is disoriented and having to be weaned off being over medicated. My mother is addicted to oxycodone and having withdrawals from other meds they tried, shame on the invisible dr. S
