I would strongly caution anyone considering Orange Park Rehabilitation & Nursing Center based on our family’s recent experience in April 2026. My brother was admitted for care and, within hours, was left in respiratory distress without timely suctioning, and his feeding tube was not even connected, leaving him without nutrition for an extended period. Despite repeated requests, staff response was delayed and inconsistent. Throughout his stay, we experienced ongoing issues with call lights going unanswered, lack of basic hygiene care, and unprofessional, dismissive communication from staff. On one occasion, a staff member spoke to him in a demeaning manner rather than addressing his concerns. Most concerning was the lack of urgency during a medical emergency. When he experienced severe respiratory distress, staff response was delayed, and emergency services were only called after persistent family advocacy. He was later hospitalized, where the findings indicated a severe case of pneumonia. No family should have to repeatedly advocate just to ensure basic, timely care for their loved one. This experience raised serious concerns about patient safety, responsiveness, and overall quality of care. I hope the facility takes meaningful corrective action to prevent this from happening to others.

Wish I could give negative stars! Stay away from this place! They come up with excuses not solutions! My loved one has been here since thursday was told she cant get out the bed only for rehab! Then told 3 other things no one not anyone in this place was able to give me a reasonable answer on why she was told this! The food she was told they have a menu to order then told they have no menus! But you can request what you would like, but when you call a nurse or somebody to request what you want.Nobody shows up. The roommate in her room, the other night was crying and asking for helpWhile there was 2 people in the hallway, chit-chat and having a good old time laughing about the lady, crying and screaming in the room.Not sure what her issue is, but nobody should be laughing at a hallway.While somebody is saying help , if somebody out there find out what the problem is and go help the person not stand in the hallway , right outside the door laughing and and chit chat and being on your phone. This place is awful.Do not send your loved one here.You'll wait hours for some water.You don't like what they give you for lunch.You'll wait hours to get another plate sent. The only person in this place was the nurse that I have spoken to so far that has actually cared to find out what medicine she's on, making sure it's the right amount, but as far as everybody else in this facility, the aides that walk around, they'd rather sit there in the hallways. And watch the tablets than actually help the patients. Meanwhile, my sister-in-law has asked for a wheelchair for 3 days in a row now. She still hasn't gotten the wheelchair she.Requested, so she can actually move her legs around and have circulation to her legs.She is still sitting in a bed and it's disgusting. I'm waiting on her primary care doctor to send a referral for services elsewhere.Don't ever put your love one in this place. Something needs to happen and I guess I will follow through.And make as many complaints as I have to cuz that poor lady in her room was asking for help and nobody was giving it my sister-in-law asking for help and she's not getting it.Nobody should be treated that way.

Hi, my name is Karen Kay, my sister was admitted in this facility after a severe stroke, I am very pleased of the care that she receives from the staff, she is a very sassy lady, and at times zero tolerance of her pain, but her therapist has not given up on her and is continually working with her, they are very friendly and caring. They just had their 2025 Christmas Party and it was amazing! the families got to meet the staff, and they were all awesome, I pray every chance I get that God will bless them all for the care they provide for my sister. Thank you all at Orange Park Rehabilitation & Nursing Center!🙏🏿

Nasty. I left OPMC to come for rehab and wound care. Within hours I retained a respiratory infection. When first arrived the stench of urine filled the air. I've worked in many health care facilities in my 50 years of commercial construction. I know the standard in which a health care facility should be met. This place gets an F on my scale. When I went to the bathroom my wound needed redressed. The PCT put an adult diaper on me instead. I asked for a breathing treatment, which is prescribed by my Dr, same PCT brought a CPAP. This facility is horrible and I don't know how they can honestly think that they are treating people. DO NOT TAKE YOUR LOVED ONES HERE! One star because I cannot give less!

I am sorry to see so many negative reviews. I have not had that experience with this facility at all. My uncle was an emergency case and they took him in straight from the hospital. I am handicapped and unable to give him the type of intense care he requires. He has been there over a year now and he's very happy where he is. He likes the food and has not complained to me about any issues. They have taken him to doctor appts and to the dentist. He has received meals while I was there visiting. They were a tremendous help in finding a great realtor to sell his home. All proceeds went directly into my uncle's bank account. He realized that he needed full-time care and he was in the best place for it. The staff has always been friendly and professional. My only complaint is I wish the parking lot was bigger. I have to use a walker and once when I was there the parking lot was completely full. I had to park around the corner on grass and tree roots, which was a fall threat to me. But that only happened the one time.
