About Ohioans Home Health & Hospice

Ohioans Home Health & Hospice is a home health and hospice provider based in Perrysburg, Ohio. The agency delivers skilled nursing, wound care, physical and occupational therapy, and specialty services like Coumadin testing and palliative care to patients recovering at home.

Families describe stark contrasts in their experiences. Those who connected with specific nurses, Amanda, Ashley, Jennifer, Barbara, and Charles, consistently praise their clinical knowledge, patience, attentiveness, and compassion. One family wrote that their nurse was "personable, friendly, professional, and experienced." Families also report successful outcomes in complex care, from diabetes management to post-surgical recovery and PICC line training. Yet multiple families recount serious scheduling failures, missed appointments despite repeated calls, orders that went unfilled for days, and care cancellations. Some report being instructed on wound care by phone without the nurse ever seeing the wound in person.

The rating of 3.7 out of 5 across 45 reviews reflects this split: five-star reviews highlight individual clinicians and effective treatment, while one-star reviews focus on administrative breakdowns, scheduling mishaps, dropped referrals, poor communication about appointment times, that left patients without promised care at critical moments.

Our mom received end of life care from Ohioans Hospice in her home for 6 weeks.
Her case manager, Aurora, and many others who provided direct care, were beyond amazing. They went above and beyond showing our mom compassion, kindness, and treated her with dignity. We were so incredibly grateful to have them care for her and guide us through the most difficult time of our lives.

I was recently diagnosed with metastatic bone lesions, cancer. My doctor referred me to this place for Palliative Care. I looked them up to read reviews. I honestly thought that people were exaggerating or just not being truthful, I was wrong.
I was scheduled for an appointment. They had to change it. I was told they would get back with me with the new time. They didn't I had to call them. Then I received a voicemail they needed to change the time. I returned the call to confirm. This is where it gets interesting. The woman that I spoke with said that she would have the nurse Susan call me in the morning to confirm the new time. So, Saturday morning arrives and to my amazement the nurse did a NO CALL/NO SHOW! I called and the lady who answered acted as if she was so surprised and was apologetic. She said that she would contact the nurse and find out what happened and she ASSURED me I would get a call back. It is now Monday evening and I haven't heard a word. Not
a single phone call. No visit. Nothing. WOW! How could I trust my care with an agency who would treat me or anyone for that matter this way. I can't and I won't. I can't believe it a NO CALL-NO SHOW. How professional and caring. To say I was disappointed would be an understatement. I wish I didn't have to give a star. My health is important to me and I will not deal with an agency who doesn't feel the same.

**UPDATE 06/18/2026: This morning I saw another company car with license plate # KST 5008 get off the highway onto the Woodville Rd. exit in Northwood/Oregon without a turn signal.

Their employees apparently don’t know how to drive. 10/11/2023 around 18:49 saw vehicle with license plate GMP8646 speeding and not using their turn signal. Not a great look especially when your company name and info is plastered ALL over your car…

Desperately in need of a centralised patiebt management system. I’ve lost count of how many times I’ve had to repeat my medications the last month. Also a calendar for appointments like my chart to make it easier for patients to update their phone calendar.
