Absolutely unacceptable with the living conditions on the rehab floor/unit. My father is being neglected there currently with poor health conditions. Mold in the door way. Air conditioning unit dripping on my father who is not able to ambulate or turn without assistance. No linen or gowns offered for when there are incontenince episodes with my father where my mother has to go in to help him change. And due to being neglected, now my father has a stage 2 pressure injury on his coccyx. This place needs to be investigated for possible shut down. Absolutely unacceptable. Sending their information to the nursing board, ANA, and JACO for investigation. I hope someone sees this and makes changes before that investigation goes active.

Tiana was so so sweet and very helpful and made us feel better about our Mother being there. and the nurse was also nice and helpful and made my mom feel comfortable.

My Daddy is there, it’s nice the facility could use an upgrade but the nice staff makes up for that.

Do not send your loved one here. My mother came to this location to receive short term rehab after an extensive hospital stay and radiation. Once she was stable enough, she was only supposed to be at this facility for a very short stay. She went from being stable (walking, talking, taking basic care of herself) to being completely catatonic in 3 days. She ended up passing away on day 6 at this facility. There are no doctors at this location and only one registered nurse who I only saw once after days of demanding getting my mother attention. This facility is not qualified to take care of any patient with serious health conditions. I will regret allowing my mom to come here for the rest of my life.
