Our father was well cared for the few weeks he was there. Staff are very kind, attentive and quick to respond to his needs. I wish there were more comfortable accommodations for residents and visitors. Having a comfortable common area for residents to relax and enjoy activities in would be our main recommendation. Maybe advocate for more staff or volunteers to sit with and engage residents.

1. Smells like urine as soon as you walk through the door. 2. They do not come when patients press the call button. If your family/friend is not ambulatory they will not do well at all. 3. My mother came from the hospital with double pneumonia and it seems that they fraudulently charted her vitals until I told the nurses I was aware she did not receive any vital checks (to which they said they had checked her that day). My mother is alert and oriented, she was not lying. They started checking vitals after I asked about it. 4. There was a medication error by a nurse that was talking on their phone while administering medications. They call it a near miss. She gave my mom a pill container with 6-7 pills in the evening (she takes 2). My mom said it was not correct and the nurse yelled at her to take them. She said no and then the nurse took them back, realized it wasn’t the correct patient medication and proceeded to give her the correct one. They did not give her any evening medications for multiple days after that incident. 5. There are cockroaches, the window in her room was broken and there was tile falling off of the base of the wall. This place is not properly kept up. The only silver lining was that she did like PT and 1-2 of the staff that she had. She still has nightmares about being in this place. I will never send her back there and would not recommend it to anyone else.

My Mother has been at this facility for 8 years. I visit several times a week, the facility never stinks (unless someone recently had an accident), the halls and my Mothers room are clean. The staff is friendly, always answering my questions and concerns. My Mother had a stroke 4 years ago and has been taken care of well and is content living there.

We are deeply concerned about the care my stepfather is receiving at this facility. Every weekly visit brings reports of new incidents. We had no choice in his initial placement and are working urgently to move him, but the process is taking far too long. Please, think twice before placing a family member here

My uncle was placed at Oak View from Orange Park Hospital. The first time I walked in I smelled urine. Staff was standing at nurse's desk looking at there phone's constantly. When I got him to assisted living the nurse said he was dehydrated. They got him fluids right away and he was better the next day. He said the staff at Oak View was not nice or kind to his needs. Words from his mouth " it was a hell hole". He had a bowel movement in the bed due to no assistance. As a veteran and and someone who contributed so much to his community this is a very sad place. I have so much more but I will leave it here for now.
