I do not understand how anyone who has ever been inside of this building can rate this place 3, 4, or 5 stars. DO NOT SEND YOUR LOVED ONE HERE.<br><br>In 2024, my mother went here for PT following a brief hospital stay due to a moderate injury. The facility was near to her home or rated well on paper. She did not need anything luxurious, just a week to stay someplace safe and regain strength with pain management. Prior to her hospitalization, she lived independently and was planning to move into a new apartment.<br><br>We should have ran as soon as she arrived. Her room was dark, the hallways smelled musty, and there was no working AC. The bathroom in her room was no larger than 3’x3’ — impossible for someone to use while injured— with rusting, broken fixtures. Absolutely no one on staff showed any glimmer of happiness or patience.<br><br>While here, she caught Covid and developed pressure wounds. The nursing team was absolutely horrible. Did nothing to escalate her wound care, would not help transfer her into bed at night (again, she was injured) just perpetuating the skin breakdown. The MD called her wounds superficial, and did not document them to her chart.<br><br>My mother was sent home with large, open, bleeding wounds. Social work upon discharge was a joke, providing no care coordination for home health, transitional wound care supplies for first 48 hours at home, or even a med list in a bag.<br><br>Not even 24 hours after leaving this facility, my mother was (thankfully) admitted back to the hospital due to respiratory complications from Covid, and MRSA infected Stage 3 and 4 pressure ulcers.<br><br>Selecting the Lakewood facility is truly the regret of my life. Years later, my mom is still living with medical fallout of this horrible place.

DO NOT PLACE YOUR FAMILY HERE! Family member was rushed out of facility when they needed more care, with the intention of resuming therapy at home, to over 2 weeks and counting and still waiting for referral to be completed. Left voice mails for Administrator who will not return phone calls. They absolutely don't care for about the patient beyond discharge. How would you feel if your elder family needs rehab and can't get anyone to do there job as the decline from lack of care, lack of empathy and lack of accountability is awful. Terrible!

My husband Phillip attended the nursing facility here and we had nothing but a great experience! He was well taken care of and provided for. The staff was very friendly and helped him feel comfortable about being there . They catered to his dietary needs and he was able to make a full recovery sooner than they anticipated! That is great service. Met Richard the owner and he made us feel like we were guest’s in his home. Highly recommend !!!

Responded when a family member complained about an issue, but not when a patient complained. Nursing staff and AIDS did what they could however they were so overwhelmed with too many patients and not enough help it averaged one nurse or Aid per 25 to 30 patients. I was contacted by my mother 83 years old with broken ribs on average three times a day due to them missing her pain medication missing her breathing treatments in addition their rooms got very warm which only breeds germs and uncomfortable atmosphere for patients. WOULD NOT RECOMMEND. The one main nurse for my mother accused of stealing a walker and wheelchair The Walker was lent To Us by one of the AIDS because they knew my mother couldn't walk without it. I lived about a mile away and said I would return the Walker as soon as her insurance covered it which it did and they were returned the Walker within a few days but to flat out accuse someone of stealing a walker, was totally unexceptable. The wheelchair we used to take my mother to my car. The nurse that was accusing us of stealing the wheelchair we said look at your cameras my daughter brought it back in and put it in the lobby... In addition one of the nurses who will be left nameless was rude and unprofessional and didn't know anything about her patient.
