Place is a dump, smells horrible. The floors are disgusting, there’s sheets on the furniture, residents outside in gowns. Why is there no phones in the rooms? Why was mom mil served expired milk? I was shown one side of the building which was nice but she was put on the dirty side room was small and dirty sheets and blankets had stains this place is a JOKE

I tribally gave Mission Palms a five star rating, shower, since my discharge more than a week ago, I have been waiting for my in- home therapy to begin. This far I have not heard anything from therapy. According to Mission Palms, the referral was sent. I have been calling the office every day and they have yet to return my calls. I am recovering from broken femur surgery and PT is very important. I regret having to revise my rating!

The CNA and therapy staff is extremely nice and always on top of everything. The Social Service Director is more concerned about her personal growth than patient, she doesn't answer emails, and her response after letting her know corporate and VA would be contacted was go right ahead. Do not allow them to move your love one to another room with out you packing up everything. I filled out two grievance forms for a total of $280 missing items. Never got any response. The floors in the rooms are not properly cleaned. The records for visitations are kept on paper. Have requested a history records for my father's visitors for court. Never got any response to emails once again. If you think about taking a veteran to Mission Palms I suggest to go elsewhere. The veterans are not appreciated at Mission Palms. Only see Drs in the hallway at Mission Palms never going room to room.

This is the only place I would ever go for post acute care rehab! I was there for 6 weeks. The staff was amazing! The therapy was amazing! It was so clean there & Everyone seemed so happy and friendly with each other and with us as patients! Thank you for taking such good care of me and I will always recommend you as my number one choice. ♥️
