The conditions I observed in Meadowview raise serious concerns regarding resident care, safety, and overall quality of life. Multiple indicators suggest systemic deficiencies that require immediate attention and corrective action. I visited my family member on Tuesday upon arrival I had a strange encounter with the receptionist. Her hair was in her face, and she had a mask on. I could barely understand what she was saying and when I asked for directions, she just pointed to the door. This is my first time at the facility. Luckily one of the workers pointed me to the right direction. The dementia unit is in deplorable conditions. The residents are walking around unattended. They had soil diapers. And believe it or not a resident was crawling on the floor. We finally found my family member. They were sitting in a dark room with the TV extremely loud. We bought some clothes for my family member and we wanted them to be labeled. I tried to ask someone that was sitting in the room with the residents but she was sleeping. I asked the nurse at the nurses station. Can I have a marker to label my loved ones clothing I couldn't help but notice it was four nurses behind the station on their phone and the one l asked had a bad attitude so I said never mind. We'll figure it out. My family member was extremely thirsty and hungry. We could tell my family member wasn't bathe in a while and their fingers nails were dirty .On top of that, the overall environment feels inactive and isolating. There doesn't seem to be much effort to provide activities or social engagement, which can negatively affect the residents Overall, the facility has some serious issues with cleanliness, staffing, and care. Improvements need to be made quickly to make sure residents are safe, comfortable, and treated with the respect they deserve. I'm getting my family member out of there ASAP

This facility is the pits. It's not clean. There was a dirty wash rag with feces on it in the bathroom on the sink. The floors weren't clean. The residents were roaming into the other residents rooms and no one is attending to them while wandering around in the hallway. I was very disappointed! I'm afraid of my loved one's safety.

Facility is kept clean and orderly, decorated for the seasons, and gives off a homey-feel. Staff is welcoming and friendly, shows residents care and respect. Admissions director is super accommodating and informative when it comes to questions/concerns from family members, goes out of her way to ensure residents have more than what they need. Leadership is hands on as the administrator is actively involved in day to day operations and makes a point to get to know each resident individually. You can certainly trust this facility with the care of your loved one.
