If you want your loved one to be neglected, this is the place for them. My loved one used to live here, it was a decent place for a while. But now they starve the residents, have few staff, and those who were working there, left. One meal included frozen, over cooked pizza, a cheap looking brownie...and red jello. On another day, my poor loved one was looking pale, because she didn't have enough nutrients. Many of the residents have also moved out of this facility. The amount of residents have declined quite a lot in the last year. Do NOT come here. Look else where!

Meadow Falls bought out this facility from Brookdale in Feb and it has gone downhill since. Billing is a mess, every month we get billed a different amount and when I requested an itemization 3 times, I never received it. Also requested to get a care plan 3 times and never received it. Even the hospitals have had a hard time getting responses, leaving multiple requests for current med lists. The final straw was when my mom came back from the ER with a broken arm they didn’t check on her regularly and she ended up back in the hospital with a UTI and pneumonia and now is in rehab. This facility is disorganized and the negligence of the staff and indifference of its management is a danger to its residents. Do not ever put your loved one in this facility.

Absolutely horrible facility. No staff, and the little staff they did have all resigned. Allll of them! These poor residents are not getting the care they need or deserve. Over twenty two complaints to the state and they’ve been there quite a bit. The place is a disaster and unsafe for residents and staff.

As an agency nurse, I want to express my appreciation for this facility. Having worked in various places, I can confidently say that this building stands out for its cleanliness and organization. There are no unpleasant odors, which greatly enhances the environment. Additionally, the staff is exceptionally kind and helpful. It's evident that the residents are the top priority here, as they rightfully should be.

This is a review on my sister that recently became a "family" member at the Brookdale memory care unit. She was very hesitant to go to a home ; did not want to sell her home of 30+ years. This was a tough decision for our family; was it the right time? how do we select the home? What services do we look for in the home? What's the difference in assisted living and memory care unit? We made the decision, after visiting 5 other homes, that Brookdale was our choice. We liked the services in memory care unit; the room floor plan; activities for residents , overall facility size and accessibility and the overall caring attitude of management and staff at all levels. Within a few weeks she was calling me to tell me how HAPPY she is. They keep her very active with exercise, games, music --always with the team These are exact quotes from her "I love this home and all employees are wonderful." "This was the best decision ever made for me". "I now have close friends and a best friend" "hows the sale of my home going?" "I feel so much better and stronger-" (physical activity and therapy). Well, she was right--we made the best decision for her Now she is Happy and we are Happy as well. I feel Brookdale/Clare Bridge is well prepared to meet the increasing levels of care as she progresses through the different stages of disease
