I want to start by saying that the front-end staff at this facility have generally been kind, caring, and supportive toward my mother. There are individuals here who clearly show compassion, and that has meant a lot to our family. However, my experience on the clinical and administrative side has been concerning. There was a recent incident involving my mother that resulted in her being transferred to a mental health hospital. As her Power of Attorney, I was actively involved, available, and even on my way to the facility within minutes to help de-escalate the situation. Despite this, the decision to transfer was made quickly without coordination or meaningful communication with me. Since then, I have struggled to receive clear answers or consistent communication from nursing staff and leadership, including individuals such as David Williams and Stephanie White. I have also not received any direct communication or clarity from the attending physicians, including Dr. Patel and Dr. Brisbois, which is concerning given the seriousness of the situation. While I understand that healthcare environments can be complex and fast-moving, communication and collaboration with family—especially when a Power of Attorney is involved—should be a priority, not an afterthought. I am currently working through proper channels to better understand what occurred and to ensure that my mother’s care plan, rights, and support system are fully respected moving forward. I truly hope the facility takes this as an opportunity to improve communication and coordination with families, because there are good people here—but the system around them needs to be stronger.

When I first got there I thought it was good, I couldn't get a ride to get none of my things from the house so I did it on my own. I was prepared to leave the place because I start seeing things inside that didn't supposed to be happening. So to me it's not a good place to have your relative friends in to stay short or long term. Be careful.

My Name is Bruce Bean Today, I am leaving for my own apartment, and I want to take a moment to express my deep gratitude to the staff. They have been helpful every step of the way, treating me like a member of their extended family. They have guided me away from negative habits and always encouraged me in a positive direction. I would highly recommend this nursing home to anyone seeking a place filled with care and compassion. Even though I’m moving on, I plan to return often to visit and see if there’s anything I can do to help. Thank you all for everything. — Bruce Bean

I've been a resident here for 1.5 years & I am very grateful. Miss Stephanie & Miss Nyeka really helped me through one of the most difficult times in my life, the passing of my mother. I don't know what I would do without them & all of the staff here.

As I’m standing here in the middle of the hallway, there’s smoke in the air while the elderly are exposed to this and are eating. Rather than being handed their utensils one of the nurses is simply letting them fall from the plastic encasing and letting them fall next to them. My family member has fallen and had many incidents in this place and we cannot help but feel his health deteriorating and is dying. On one of these occasions we asked the social worker what happened and she told us to call family members to see what was wrong. This is unacceptable and highly unprofessional. While I understand there are people here who work and give their best, I see on many more occasions that there are distractions, lack of professionalism, and no necessity to change.
