I didn't want to even give this place one star. If I could give negative stars I would. In the very short time my dad was there the care I witnessed my dad and other residents receiving is appalling. Several residents told me stories of mistreatment and falling because no one came to help them. My dad arrived on a Friday afternoon. Apparently, the staff doesn't read paperwork. He was nothing by mouth and I had to stop 2 separate employees from giving him a drink and an entire plate of food. I found my dad hanging out of his bed one day yelling for help. Was told by the staff they couldn't help him because he had not been evaluated yet. They are EXTREMELY unstaffed. The wait for after pressing the call button was typically one hour! After a hour an aid would could and say turn the call light off I will come back. Most of the aids are lazy, rude, and not very gentle with the residents. Scolding them for pressing the call button. On one occasion I asked an aid to help sit my dad up in bed and was told, " He's just going to slide down anyway." I was helping my Dad with things this facility was being paid to do. A family member was there for an average of 10 hours a day too. If this facility is supposed to be above average I would hate to know what goes on at an average rated one. I understand my dad was not the only resident there who needed to be cared for, but this was ridiculous. When he arrived the nurse told me response time for pressing the call button was 15-20 minutes. Please think twice when you are faced with putting yourself or loved one in this facility. I did leave a message for the director to contact me and of course no call back.

This is the second time my dad has been here for rehab this year. He has good comments about the food, physical therapy and staff. He’s been at other rehab places in the past which he has complained about. So we are relieved and glad he is giving positive reviews about this one!

I would give this place negative stars if possible. Mom has been there for rehab for 4 weeks now. First day there, CNA comes into her room to clean her up and is holding a conversation on her cellphone while cleaning mom up. And then proceeds to tell my mom to help her because she's killing her back. Let's define Linden Grove's rehab...Sitting in a wheel chair in a group of 5-6 hitting a balloon back and forth between other patients, passing a wooden block between patients. Seriously??? Her legs are weak and she's supposed to be getting one on one therapy to improve her leg strength, not her arms. Food is cold all the time and nasty. She's received someone else's tray several times. Call light goes unanswered for over an hour numerous times. We've had to call the nursing desk to get someone to her room to assist her several times. Other times they answer the call light and tell her they will be right back and nobody comes back. A pill was on the floor under her bed and a nurse asked her if she wanted it? Thank God my mom's brain is alright and was able to tell the nurse, " No thanks, that pill has been there since I arrived." So that's another issue, how clean is this place? My mom said she has never seen housekeeping in her room, and yes she is with it and awake all day long. She takes Lasix daily for years. Her legs had increased swelling and they supposedly doubled the dose for a couple days with no improvement. Went to ER for an evaluation, got a double dose of Lasix there, went back to rehab, and what do you know, swelling instantly better overnight, 10 pound weight loss. 2 days later another 10 pounds down. Were they actually giving my mom her normal daily dose? Doesn't seem like it. And recently mom developed a sore on her ankle from not being repositioned frequently. A cream is supposed to be applied daily along with a fresh bandage. That hasn't happened daily. Several of us in the family have called to talk to the manager. Left her numerous voicemails with no response. The only person there that seems to care and know what they are doing is the social worker Lauren. She has done her best to address our needs and concerns. Thank you Lauren. Unfortunately, we originally applied to several rehab centers and this is the only one that accepted mom. If you can avoid this place, please do. I feel this place only attempts to make your loved one's condition worse, just to push them into a nursing home and make more money.

A family member stayed here for rehab and during their stay we were told that they were doing such a good job and improving so much, that the insurance ended up cutting their stay short.. but upon returning home they could barely walk without assistance, use the bathroom without assistance, and had a new wound that no one at the facility knew about it. The discharge paperwork did not reflect the reality of the situation and my family member was unwell and unstable on their feet, it was incredibly unfortunate. After a week at home, my family member fell and passed a day later. I don’t know what happened to lead to their abrupt discharge but it was all and all a series of unfortunate events.

My mother-in-law is 92 and has advanced memory loss. She declined very quickly and we needed to move her to a community that could provide appropriate care for her. We had investigated Linden Grove New Berlin and other communities about a month prior to needing to move Mom. We were so happy with the Linden Grove environment, available services, that we were treated so respectfully, and that Linden Grove could accommodate our urgent need. We were able to move Mom very quickly at the end of December 2024 and are so pleased with how she transitioned easily and without much agitation or worry. The staff truly cares about each resident, making their final journey loving, dignified and respectful. The nurse and health care providers keep in close touch with us, making sure Mom stays as healthy and happy as possible. Although it's never ideal or easy to transfer someone you love so much to a memory care community, we really feel we made a great decision with Linden Grove. Meals are great, activities are plentiful, cost is appropriate and we've been fortunate to meet and get to know most of the other residents. Nothing is every perfect, but Linden Grove is among the best for caring for loved ones.
