we arrived last night and I was a little disappointed in the lack of communication. However, the charge nurse on the third floor was lovely. Gave me directions to get home in 10 minutes and when I came in today, I was so impressed your entire staff is so helpful and caring and responsive to your patient’s needs. The only issue I have is my husband is a very large man and he cannot sleep in that wheelchair that he sits in and he used to napping two or three times a day if he had a recliner, he would be more comfortable, but I didn’t see any in the J room and that is the only issue I have the food what looked lovely I didn’t needed it. It was his dinner, but the fish look beautiful. I like the fact that he gets a little sweet every day. I like the snacks and I love the constant attention to make sure that everybody everybody in that day stay hydrated, and that your staff connects with everybody that seems to need some help.

I do not recommend this facility. My father is currently at Life Care Center of Lynn, and our experience has been very concerning. There have been multiple times where he has called for help and has been ignored. I have personally been on the phone with him while he was calling staff with no response. Today, he had an appointment and was not prepared on time, and he was not even given breakfast. This is basic care that should never be missed. I am also very concerned about hygiene. No patient should be left in a soiled diaper for long periods of time. This can lead to serious infections and bedsores, and it is unacceptable. While some staff members are kind, others lack compassion and professionalism. The case manager has also been very unprofessional, and at times her behavior has felt discriminatory. At first everything seems fine, but over time the level of care is not consistent. Families need to check on their loved ones often. I am sharing this so others are aware and can advocate for their family members.

My mother was sent to this place post influenza hospitalization in January of this year for one week of physical therapy in five days, she received one short walk down the hallway with her Rollator walker that was the extent of the physical therapy given. On the second day, there she began experiencing severe diarrhea. I alerted the staff multiple times. They did nothing to rehydrate her or further test Her electrolyte levels or kidney function. As a patient with congestive heart failure and chronic kidney disease and type two diabetes their clinical failure was profound. After three days of diarrhea and constant neglect my mother was swelling up like a balloon. I requested they call an ambulance as I live out of state, they allowed her to refuse the ambulance as she did not want to go back to the hospital, even though she clearly needed to go back to the hospital. This happened twice. On the fifth day I finally escalated it to the highest level with the director and nursing supervisor and we got her back to the hospital where her kidney function had went from a baseline of 45 prior to her stay there down to 13! Since then she has had three more hospitalizations due to the acute kidney injury sustained at this facility. It is completely unacceptable. Additionally, their nurse practitioner Tshilanda Mukala is completely ignorant and rude to family members and hung up on me. She also confused my mother with another patient it turned out. My mother caught C diff from this place. Do not send your loved ones there!! Total clinical failure of care all around, sadly at 87 my mother’s decline since this experience has been precipitous. I have since spoken with a former RN who worked for them and she said the place should be shut down. She quit to avoid jeopardizing her nursing license. If your loved ones must receive residential nursing care or short term rehab literally go ANYWHERE else!

My mother recently stayed at this facility for rehabilitation, and we had a very positive experience overall. The communication from the staff was excellent—they kept our family informed, answered questions promptly, and made sure we understood each step of her care. We’re very grateful for the support she received and would confidently recommend this facility to others in need of rehab care.

There are some great employees there but many terrible ones. Hit button at 2am to use commode. Takes an hour and she asked what I wanted. Told her and she said she was busy and I would have to wait until I threatened to wet the bed. This is one example. There are many others. Food was disgusting. Who takes a grilled cheese sandwich and puts veg medley with liquid in it and puts it on same plate. So many meals like this.
