My mother was placed here for rehab. In the hospital she was a 2 person assist to the bathroom and was ordered physical therapy every day. In the 2 weeks she was there, no one assisted her to the bathroom. The call button didnt work, the safety grab bars fell off the wall if touched and there was no lights outside the facility. She did not feel safe. My family and i stayed with her majority of the time. When i asked for a transfer, the administration asked for 24 hrs to rectify the issues. They discharged her immediately saying she was at her baseline. She went home with no services in place. 2 weeks later she was back in the hospital for the same issues. This place was an absolute nightmare. What we saw will haunt me. I wish all the patients had family advocating for them. It needs to be investigated and shut down.

I am so so sad to say that my mother passed away while in this horrible place in 2015. I needed her to be near where we lived and after inspecting the location I chose keystone, so I could get to her in a hurry if needed. I was her advocate and it worked for a while. I have too many details and not enough room to write. I will never forgive myself. I’m so so sorry Mom. It’s 2025 and I see complaints are the same if not worse. Find somewhere else if you need to place a loved one. But keep them close for as long as you possibly can. ❤️‍🩹☮️🙏

One of the individuals I care for needed a short stay in a more hands on setting. The whole team was great from start to finish. Rebecca P helped so much throughout the whole journey. Discharges can be difficult with all the med policies; they set us up for success when it was time for him to come home. Thank you for eveything!!

I am adding an update to our review of Leom Rehab and Nursing. My brother has been here for a little over a year and he has had extremely good care! Every staff member we encounter is very helpful and patient. The administrator (Michelle) has been available any time we ask to see her. The CNA’s do a great job with our brother. They have a very difficult job and they perform it well, even when understaffed. Rebecca (social worker) is still there and still wonderful to us! The nurse staff is very attentive and they notice everything, which gives us comfort. Our brother has fit in well and seems happy. Diane (activity director) makes sure he is involved in all the activities which he enjoys. So if you need a caring facility we wouldn’t hesitate. Again, this is just our experience. We unfortunately had to place our brother in a nursing facility for long term care in April 2024. Leominster Rehab and Nursing was able to admit him. He has only been there a short time, but our experience has been fairly positive. They are very understaffed but we are there visiting every day. We have had the opportunity to interact with numerous workers on a regular basis, most of whom have been very helpful. One in particular is Rebecca Powers the facility social worker (who I will give 5 stars). She has been extremely receptive and answers our questions thoroughly. We see her roaming the building often and makes herself available to interact with us and my brother. Nursing homes are an emotional place to have to place a family member but it makes us feel a little better that Rebecca has been so helpful.

I had a brief experience at Leominster rehab. Some upgrades needed in the building. The social worker is amazing. Rebecca was always available for questions and support. Best social worker I have ever met. Also my nurse Leslie was so good and compassionate. Highly recommend
