Jefferson Nursing and Rehabilitation Center is a 120-bed skilled nursing and long-term care facility in Beaumont, Texas. The facility serves patients in both rehabilitation and respite care settings.
Families' experiences at Jefferson have been notably divided. Several praised specific nursing and therapy staff for attentiveness and competence, with one family noting the facility felt "modern and clean" and staff "check on her frequently." Others highlighted the dedication of individual employees, with one family writing that staff "go as far as giving out their cell phone numbers so we may reach them even after hours." However, multiple families raised serious concerns about inconsistent care standards: reports included unattended call buttons, infrequent response times, lost medications, inadequate cleaning of patient areas and bedding, and insufficient family orientation upon admission. A small number of reviews documented preventable incidents, including falls and delayed medical responses. These contrasting accounts suggest the quality of care may depend significantly on which staff members and units are involved in a resident's stay.
Jefferson holds a 4.2 out of 5 star rating based on 72 Google reviews. Prospective families should plan to visit in person, ask detailed questions about staffing assignments and care protocols, and maintain active communication with facility leadership during a resident's stay.

My mother was a resident of this facility for a couple of months and I was very impressed by Nurse Cheryl and Nurse Shana who did an exceptional job caring for my mother, as well as Sam in the physical therapy dept. Ms Shelly in admissions really helped me out as well and made the admissions process very simple. My only concern was not getting my mom's mental evaluation done alot sooner so her mental health medication could be adjusted to help her with her anxiety/ mood disorders. I do recommend this facility to anyone who may be seeking care for them selves or a family member or friend. Thank you to the entire staff for a stellar job.

When I toured the facility for my mom, I was greeted with smiles and everyone was helpful. While touring I met some of the nurses and they seemed very nice and down to earth. The facility was clean and organized and some of the mobile residents were playing games. I loved the intimate feel to this place!
