Nursing and Aide service is not good... hopefully it is because the facility is under staffed or something. Multiple instances when they didn't have patients assisted or dressed by 9am and therefore not ready for PT appointments! Medication not given on a regular time schedule. However, the PT staff were helpful. As this facility is part of Yale issues of bad service, Inconsistent schedules and unprofessionalism shouldn't occur regularly as it has. Unfortunately my overall recent experience having a family member there was not very good!

My Dad is at Grimes now, and while the staff have been lovely and communication has been great, I am not sure its the best place for people with Cognitive Decline who are likely to get out of bed. My Dad just had an "Unwitnessed fall" because the beds have low rails and Dad has tried to get out of bed before. It's a little frustrating that there isn't better monitoring and safety to make sure falls hopefully don't happen.

My aunt was recently there for 10 days for rehab they made it as enjoyable and comfortable as it could possibly be. The nurses were attentive they responded quickly to anything you needed the place is very cleaned the floors were cleaned 2 or 3x a week they were very very friendly to the visitors even letting us wait inside for our ride. They did a lot of exercises and rehab overall a fantastic place I would give it 10 stars if I could.

I am writing to formally lodge a complaint of Racial Bias regarding an incident that occurred at the Sister Anne Rehabilitation Center on December 17, 2024, at approximately 4:00 PM. As a six-months-pregnant individual and a school bus driver, I stopped at the facility to request permission to use the restroom. Upon entering, I was greeted by a woman named Melissa, who appeared to be an older caucasian woman, likely in her mid-60s. I explained my situation and politely asked if I may use the restroom due to the urgency of my condition. To my disappointment, Melissa denied my request, stating that I could not use the restroom and explicitly disregarded my pregnancy. When I asked to speak with a supervisor or another staff member, Melissa informed me that everyone else was gone for the day. In disbelief I asked for her name and wished her a good day, she did not respond until my back was turned walking towards the door. I exited the building through the same door I entered, disappointed and still in need of a restroom I hurried across the street to a corner store and the gentleman told me their water wasn’t running. I had no choice but to continue driving for another 25 minutes to return to my workplace, where I could use the facilities. Abruptly after, I was called aside by my manager, who informed me that the supervisor from the Sister Anne Rehabilitation Center called to report that I had been rude, aggressive , and I have broken their door. This left me shocked and appalled because I am a professional and carried myself professionally and although I was lied too and told no one else was there I did not change character, so how could a supervisor call with these false accusations (Was this an attempt to get me in trouble and affect my livelihood). I requested to obtain video footage from the facility, which would clearly show that I exited the building calmly and in the same manner as I entered.I am deeply concerned about the discriminatory behavior I experienced during this incident. Melissa’s refusal to accommodate my reasonable and urgent request, coupled with the false allegations made against me, raises serious questions about her conduct and attitude toward others.I respectfully request that Melissa’s actions be thoroughly investigated, including a review of any prior complaints that may have been lodged against her. Such behavior is not only unprofessional but also deeply insensitive, particularly toward someone in my condition.Thank you for your attention to this matter

I love this place, I was there in 2013-2014. I stayed like 3 times, different times in a period of 2 yrs. I had major knee surgery and had some complications with surgery. My stay there was so very pleasant. The staff was to friendly and they really looked out for your well being. I always recommend my folks to go there if they have the option. It’s local, very clean and the food is really good. So my thumbs up and 5 stars for Grimes.
