Levels of Care we Offer

Assisted Living

Assisted living is the perfect option for older adults who need a little extra care but want to maintain a fun and social lifestyle.

After move-in, your loved one will have access to skilled caregivers who provide everything necessary for a meaningful, elevated experience.

An Elevated Assisted Living Experience

Aging is part of life for everyone. We slow down as we get older, and common household activities gradually become more strenuous. When they reach a breaking point – whether due to medical concerns or daily activities – assisted living services are necessary.

At Frontier, our communities offer exceptional assisted living services for older adults who need a little extra care but want to maintain a fun and social lifestyle.

After move-in, your loved one will have access to skilled caregivers who provide everything necessary for a meaningful, elevated experience.

This is all possible through our Montessori-inspired culture. We believe engagement, socialization, and inclusion are the three most important factors in a life well-lived, and we strive to bring them to our assisted living residents 24/7. Our staff will consider every last detail of senior care to ensure you or your loved one fall in love from the moment you walk into the community.

What is Assisted Living?

Assisted living is one of the levels of care many retirement communities and nursing homes across the country offer. It’s an excellent fit for many senior citizens and mature adults who require a little extra help with their day-to-day activities.

Access to care staff is where the value truly lies in assisted living. Residents living in assisted living facilities live in their own private rooms while receiving extra help with many medical services from specialized care providers. These hospitality and healthcare staff members monitor residents’ well-being and help keep them healthy.

Our wellness services are robust. They include everything from convenient access to skilled nursing care to activities of daily living (ADLs) – whatever is necessary to maintain a safe and secure living environment. But not all residents are the same. The right Frontier Management senior living community can tailor their services to fit your loved one’s individual care needs.

Frontier assisted living communities provide medical care and residential care while maintaining an active and engaging environment. That includes anything that makes life easier, such as:

•    Laundry services & Housekeeping
•    Personal Care Services
•    Medication management
•    Recreational activities
•    And much more

Most importantly, our staff focuses on increasing and maintaining quality of life as much as possible. We’re 100% committed to this. Our staff ensures each resident receives round-the-clock attention and hands-on continuing care as well as routine access to vibrant social events.

Memory Care

Our community specializes in Memory Care for residents with Alzheimer’s Disease or Dementia-related illness.

Through our award-winning SPARK program, your loved one will receive round-the-clock care that encourages a happy, fulfilling lifestyle.

Engaging and Purposeful Memory Care Communities

When a loved one receives dementia or Alzheimer’s disease diagnosis, it affects the entire family. Dealing with gradual behavior changes and increasing medical needs is difficult, and that stress can worsen when long-term care options become necessary.
It’s our mission to relieve that stress from your family.

At Frontier, we believe the process of selecting a new memory care community for your loved one should be an enjoyable one. And while memory illness presents incredible challenges to people worldwide, there is still an opportunity for a rich and meaningful lifestyle with every resident.

What is Memory Care?

Memory Care is a specialized level of care that focuses specifically on older adults with Alzheimer’s, Dementia, or other memory-related illnesses. Residents live onsite in an assisted living community where they have 24-hour access to a wide range of care options. Whether they need routine access to healthcare professionals, help to complete day-to-day activities like bathing and transferring, or both, they are taken care of in a memory care community.

Access to caring staff is where the value truly lies in senior living. Residents living in memory care units receive extra help with many different tasks from specialized caregivers. These hospitality and healthcare staff members monitor residents’ well-being and help keep them healthy.

These services are robust. They include everything from convenient access to skilled nursing to activities of daily living (ADLs) – whatever is necessary to maintain a safe and secure living environment. But not all residents are the same. The right Frontier Management Memory Care Community can tailor their services to fit your loved one’s individual needs.

Many of these services are similar to assisted living facilities, but memory care emphasizes developing and maintaining cognitive functions. At Frontier, our caregivers go above and beyond to ensure your loved one is not only safe and secure but also living a happy and purposeful life.

We believe memory care residents should live in an environment that:

•    Encourages autonomy and self-expression
•    Provides a structure which supports memory retention
•    Ensures each resident’s healthcare and long-term care needs are met

Most importantly, our staff focuses on increasing & maintaining quality of life as much as possible. We’re 100% committed to this focus. Our staff ensures each resident receives round-the-clock attention, hands-on senior care, and continuous access to engaging social events.

I honestly would not recommend coming here or bringing your loved ones here. The staff tells nothing but lies to the families just to lure you in . Once you’re in it’s all downhill from there, my mother is currently staying here and she has not been accommodated with her needs, they pm shift med tech is never around or she is always sleeping in they office, the care staff will not help with my mother only 2 ladies go over and above for my mother and when they do that they get in trouble, since this new company “NorthStar” came and took over nothing has happened. The management does not listen to the residents requests, the maintenance man does absolutely nothing but sit in the manager office all day flirting . All of their good care staff is leaving and I see why they get treated like crap. Other care staff don’t even laugh or smile or interact with the residents . It’s really bad and not a happy setting .oh yea they wana remove our honest reviews just to keep it from looking bad ! The last company was sued for a wrongful death (public records) and now this new company is stepping in and they are going to be facing some lawsuits as well. It’s just so bad there smh. Nasty attitudes the management only cares about how they look ! Nothing gets done

I would not recommend this facility to families seeking quality care for their loved ones. Based on my experience, there appears to be a significant lack of communication and transparency with residents’ families. My mother is currently a resident here, and many of her needs have not been adequately addressed. The evening shift medication technician is often unavailable, and there have been occasions where staff members appeared inattentive to resident care. Only a small number of caregivers consistently go above and beyond to assist my mother, yet they seem to receive little support or recognition for their efforts. Since the new management company assumed responsibility for the facility, I have not seen meaningful improvements. Concerns raised by families are often ignored, and management has been unresponsive to requests regarding resident care and quality of life. It is also concerning to see many of the facility’s strongest caregivers leaving. Given the work environment and the way staff appear to be treated, it is understandable why turnover is so high. Additionally, some staff members seem disengaged, showing little interaction with residents, which contributes to an atmosphere that feels unhappy and unwelcoming. Overall, this facility has not met our expectations for resident care, professionalism, or family communication, and I would encourage others to carefully consider all options before choosing this community for a loved one.

If you are considering placing a loved one at Greenhaven Estates, I strongly encourage you to research the facility carefully and consider all available information before making a decision. According to publicly reported court records and news coverage, the facility was previously involved in a wrongful-death lawsuit involving a 100-year-old resident who wandered outside from memory care and later died from hypothermia. A Sacramento jury reportedly awarded significant damages to the resident’s family. Information about that case is publicly available and worth reviewing when evaluating the facility. Separately, my family had our own experience with the facility while my aunt lived there for nearly two years. During that time, we personally observed several issues that concerned us. At one point my aunt experienced a fall in her apartment and, based on what she and our family were told, she remained on the floor for several hours before assistance arrived. Afterward, there was still visible blood on the flooring that, from what we could see, had not been properly cleaned. Two months ago my aunt suffered a stroke and had to be transferred to a higher-level care facility. Her partner still lived in their two-bedroom apartment, so rent continued to be paid while we worked through the transition. When family members returned later to collect my aunt’s belongings, we found that the bathroom had not been cleaned since she left. There was still waste in the toilet that appeared to have been there for an extended period of time. Seeing that condition was very upsetting for our family. There were also communication issues with administration regarding the required 30-day move-out notice. My father was initially told by a staff member that the notice had already been handled, but later the acting administrator told him it still needed to be submitted in writing. This created confusion and additional stress during an already difficult time for our family. While we were at the facility removing my aunt’s belongings, we had an uncomfortable interaction with a staff member. The tone of the interaction was confrontational, and during the exchange I was called a derogatory slur referencing my sexual orientation. That experience was extremely upsetting and not what anyone expects when dealing with a facility responsible for caring for elderly residents. Throughout the remainder of the time we were there, several staff members came by repeatedly while we were moving items, which made the situation feel tense and uncomfortable. Based on these experiences, our family ultimately decided to move our loved one to a different facility where she could receive the level of care she needs. Every family’s experience may be different, but ours raised serious concerns about cleanliness, communication, and professionalism. For that reason, we are also submitting our concerns to the appropriate California oversight agencies so they can review the situation. Choosing a care facility for a loved one is an important decision. I strongly recommend visiting facilities multiple times, asking detailed questions, and reviewing public records before making a choice.

The 3 stars are because the place is clean & staff have very kind & patient with my mom. However 0 star for the food & administration. My mom is in the memory care side but she's the quite lucid. The food is abominable. Mom has had food poisoning 2 in less than 2 months. Her medication has been lost for weeks, then mysteriously reappeared when the ombudsman was mentioned. I asked the director if she'd eat the food served there every day & she didn't respond. Again staff are absolutely lovely. If you move your loved here, please please visit often & monitor the medication yourself!
