I give the staff a 5 star but the conditions of the facility, the beds, bedside tables and furniture a 1star. The odor will take your breath away until you get adapted to the surroundings. For a facility in Fernandina Beach it is less desirable than any facility I have visited in our surrounding area. I wish for my family to be in a more respectful facility.

My brother was a patient in the long term patient area for 9 months prior to his passing. He found the staff to be friendly and compassionate. I want to especially thank three employees. Naslene in the Business Office for her professional help with helping us to understand what his financial obligations would be. She explained the workings of social security benefits and how the "system" works. She was very kind and respectful to my brother. Tammy in the Activities department was such a huge help with these things too. Jeremy was my brother's nurse. What can I say?? He was absolutely wonderful to him and became a dear and close friend to him. The staff is very kind overall. The facility especially in patient rooms should be cleaned more often and food is not very good.

I am very pleased with several nurses in rehab. Kathy, Amanda, and many others are compassionate and competent. Cole at the front desk is amazing. I had an awful experience with Sereana in social sevices. I was told,point blank, she did not have time to provide me a list of long term hospice care facilities for my loved one, after she promised to do that 4 days earlier. She said it was my problem and she did not have a bed for him , so I would need to figure it out because Medicare was going to stop paying. She was rude and said, “Use your phone, it’s a computer.” Nasaline was also rude and provided one option. Today, I was treated with respect by Latoya, in social services. She provided me with the information I needed to move forward and a huge weight was lifted.

I am extremely dissatisfied with the care my family member received at this nursing and rehabilitation facility. What should have been a place of compassion and support turned into a very upsetting experience for our family. One staff member in particular, Darian Turner, was incredibly rude and disrespectful. During the 3–11 shift last week, she told my family member that she would come in “whenever she can get to them, if she can,” which is completely unacceptable when someone is relying on timely care and assistance. No patient should ever be made to feel like they are a burden or not a priority. What’s even more concerning is that this behavior was not a one-time incident. She returned the next day and repeated the same dismissive and unprofessional attitude. This pattern of behavior shows a serious lack of accountability and compassion. Overall, I am very disappointed in the level of care and the actions taken by staff. Patients deserve to be treated with dignity, respect, and urgency—none of which were demonstrated here. I would not recommend this facility based on our experience.

I want to give a shout out to Ophelia, my father’s nurse, and the rehab PT team. My father talks very highly of Ophelia. He found her personable, hard working and attentive. He really enjoyed her as his attendant. He described the rehab team as professional, personable and effective. These are the highlights of his experience in your facility. I hear the quality of food is lacking greatly and listened to constant complaints regarding this issue. Overall a positive experience with staff, thank you.
