My grandmother had dementia and cancer they treated her with immense respect and care they were simply fantastic. The staff and administrators were peerless in making the process as friction-less as possible.

My Father has been a resident here for 6 months. In that time, there have been 5 instances of the staff not providing him the oxygen that he needs. One of these incidents resulted in a trip to the hospital. The Administrator and Director of Nursing are very difficult, defensive, and demeaning to work with if you have a care issue.

The staff and administrator here care so much about their residents. I cannot recommend Concordia enough. My mother has had a wonderful experience and it is always clean when I visit.

My husband was banned from this facility because he was too much trouble after a short respite stay. He is a 100% disabled Veteran with dementia. This facility contracts with the VA! I was told not to bring him back for long term care either! This is not a very nice place anyway. It is old, dingy, and I felt concerned but they had other Vets there. They are definitely not a very professional group.
3 inspection visits in the last three years, 45 citations on file. Inspections are conducted by state agencies on behalf of the U.S. Centers for Medicare & Medicaid Services (CMS).
Have a registered nurse on duty 8 hours a day; and select a registered nurse to be the director of nurses on a full time basis.
