My old lady neighbor had a terrible fall outside on her porch. Community and their staff got her back to 100% so quickly!-

The only skilled nursing home, I Recommend to families when discharging from The hospital.. The administration staff is easy to work with and coordinate discharge plans even on weekends!

Our biggest regret. Basic needs not met. They left our loved one to sit/lay in same position for hours or even the entire day so he began developing pressure wounds. Left to sit in feces and urine hours after notifying CNA and/or nurse. Several hours would go daily by with no check-in from nurse or aid. Some days food tray left out of reach. Help eating not provided so missed meals when we weren't there. Dentures never removed or cleaned leading to pneumonia. Family had to take shifts to provide basic care. Found out at one point no therapy had been done for weeks, the whole point of him being in rehab for a broken vertebrae. When I asked nurse Susan she simply glared at me saying, "do you want him to hurt? Are you trying to hurt him?" Therapy finally initiated. Patient was memory impaired with spine fracture but physical therapy put him on toilet and left room, leading to a fall and hip fracture. Nurse Susan cold to family/patient and told them it was "patient's own fault" for not waiting for assistance. His memory was so poor he was unable to retain what what he was told even 2 minutes prior, obviously not a candidate to be left unattended in a potentially dangerous situation. Family members witnessed him being roughly rolled onto wound vac and aid laughed when he howled in pain. There are a few wonderful CNA's, but not enough to change the culture or quality of care. Shortstaffed facility. Caregivers don't have enough time to complete basic care so they mark off things like baths being given when they were not. A whole family taking shifts, being present, providing care, submitting complaints to management, and advocating for him still wasn't enough. Too many more grievances we encountered to mention. I'd rather die alone at home than ever have to live like this.

My mom was a resident there for 4 years and passed away in July. Still haven't had a single call from ANYONE at Community to express condolences. I don't have the emotional energy to write out all the terrible experiences she endured. I would have tried to move her but was told they were the "best in town" by an outside professional.

Therapist are great and work for you to get better. The nurses are very good and professional. The laundry stuff are very nice and they are very fast. They are very helpful if you lose something and even speak in the hallway calling you by name. Last but not least CNA. The CNA work hard even on days that they may don’t feel good. But they are always professional and do what needs to be done to keep healthy. This is my second time here and if I have to go to a rehabilitation center I will ask for Community Health and Rehab Center
