Commonwealth Home Health has been my go to agency for Home health referrals for about a year now. The referral process goes by so smoothly and we work great together. They are always able to meet the needs of my patients that I am discharging back to the community. The communication between me and this agency has been excellent as well and they provide me with post updates of how my patients are doing after discharge. They also provide excellent nursing care and therapy services to my patients with low re-hospitalization due to their patient centered care and various programs they offer.

The last two weeks have been a really trying time for my family because of the unknowns regarding the health of a loved one. There have been too many phone calls to count between talking to doctors, nurses and our case manager at the hospital. When the time came to make the decision to bring in home health care, I wasn’t looking forward to dealing with anymore people. However, the first phone call I received from Commonwealth Home Health Care was from Alicia. I cannot say enough about her knowledge and mostly importantly her kindness and compassion. I would recommend dealing with her to anyone who finds themselves in this awful situation. She really makes you feel confident that your loved ones will be in good hands.
