My mother in law was a resident from 2021 up until April 2026 when my wife and I discharged her under what we believed were emergency circumstances. Once at home, we discovered severe skin breakdown on her bottom and pressure wounds on her knee and heel.

I rescued my loved one from this hellscape last month because my private caregiver witnessed my mother sliding down a lift bc the nursing home CNA was too busy cursing about staff shortages to pay attention to what she was doing. My caregiver was able to prevent my mother’s head from hitting the support beam. By the way, that CNA still worked there when I discharged my mother.

From the moment we walked in, we felt welcomed and reassured that our loved one would be in good hands. The facility is clean, well-maintained, and has a warm, homelike atmosphere. The nursing team goes above and beyond to provide compassionate care, and the therapy staff is top-notch—always encouraging and helping residents regain their strength and confidence. Communication with the families is excellent. They keep us informed and involved, which brings us peace of mind. It’s clear that the residents are treated with dignity and respect. I’m grateful for the support and professionalism of everyone at Columbiana Health and Rehab. Highly recommend this facility to anyone looking for quality nursing care in a compassionate environment. Being able to volunteer my time at Christmas and play the grinch is always a joy.

CHR has a clean facility with an upbeat staff that interacts with the residents regularly and nurse's constantly check on the residents to meet their needs. They are one of the best facilities I have seen that is more home based. The staff are like extended family members to the residents. The love and care I have seen the staff provide the residents is outstanding.

My mom is staying here to get well enough to come home after having back surgery. The staff has been very helpful and communicates with me often. The decor is a little dated however it’s still nice. I would suggest this place to anyone.
