My uncle was admitted to the SNF for rehab and physical therapy amongst other issues. If I could I would give no stars. The administration of this facility should be ashamed of themselves, and the way they allow this SNF to run. It is clear your real priority is the money you rake in off the backs of the elderly. The staff hired is clearly miserable and could care less if people need assistance. The call bell is going off constantly and it is not for "different" rooms as they try to say. They IGNORE people in need of assistance while they stand around laughing, socializing putting on makeup and smoking outside at this "smoke free facility" We waited hours after his admit for someone to come in and assess him/greet him give us some information, and NO ONE ever came in after getting paperwork signed. Never checked on him the entire night, and then lied to our face the next day when we asked why no one helped him get into bed, go to the bathroom or provide him with needed wound care. The truth is no one is in charge, everyone is passing the buck and no one cares including the owners and administrators. Stay far away if you care at all for your loved ones.

The skilled nursing rehab is possibly the worst medical facility I have ever encountered. I wish I could leave a negative 5 star review. In less than one week of having my mother there, she has been denied basic care on multiple occasions and this morning she fell attempting to make it to the restroom by herself. This fall resulted in a gash on her leg. The staff is rude, uncaring, and unresponsive. Friday night she was nauseated and dry heaving and had hit her call button multiple times to request zofran. In TWO HOURS no one came into the room. I was on the phone with her when a nursing assistant walked in and was rude, didn’t ask what she needed, said they were short staffed and walked out of the room speaking to another staff member, laughing. This is not only patient neglect, this is cruelty. To not even ask what my mother needed or what was wrong and then to mock her and laugh. I will be moving heaven and earth to transfer my mother to a different facility as soon as possible. Do not entrust this place with your loved one. I will be filing complaints with the Ohio Department of Health and CMS for patient neglect. I worked in healthcare for 10 years and never once encountered this level of unprofessional behavior and lack of compassion for patients. Truly awful to have an ill parent and know they are not being properly cared for.

My mother enjoyed her time in independent living at Brookdale Westlake Village. The staff has been extremely kind and helpful, including billing, food service, housekeeping, maintenance, and the front desk staff. The food was usually excellent and there are many quality activities to choose from. There is a warm and familial feeling throughout the Brookdale Village independent living. We also appreciate the flexibility of the supportive Brookdale At Home staff who were imperative In keeping my mother living independently. I highly recommend it.

OVERPRICED & UNDERSTAFFED. Independent living roof is falling apart after spring storms. Do not take your family here for therapy or long term care. Short staffed filled with agency nursing or Nursing Supervisors working down as an aide with high patient care ratios. High schoolers in the kitchen serving mediocre food. Self pay is $700 a day at the minimum. Administration does no hands on care all office. No community involvement. Illegal use of Mechanical lifts in assistant living by non tested caretakers. Recent bed bug and scabies cases. For the same cost check out Vitalia & Avenue Strongsville.

Our experience with Brookdale Westlake Village’s Memory Care Unit was deeply disappointing and distressing. My father was a hospice patient there, and we encountered ongoing problems with communication, cleanliness, and decision-making that no family should have to face. His room often smelled strongly of urine, even after repeated requests for cleaning. We struggled to get basic care coordination, including podiatry and medical follow-up. The worst incident occurred when my father—who was on hospice, sedated, and asleep—was sent to the ER for a psychiatric evaluation without proper coordination with hospice or family consent. I had specifically requested that hospice handle the evaluation onsite. The decision to send a terminal, sleeping patient to the ER in the middle of the night was unnecessary and inhumane. Later, the facility abruptly required that we hire a “sitter” for my father with only a few hours’ notice. Brookdale later agreed to reimburse us for a week of sitter costs but has been slow to follow through. Ultimately, we had to move my father out of Brookdale for his own safety due to the facility’s repeated incompetence and lack of hospice coordination. When I raised these issues with Brookdale’s internal investigation team, no corrective action was taken, forcing me to escalate the matter to the Ohio Department of Health for a formal review. While a few frontline caregivers (like Nakila and Sarah) were kind and compassionate, management failed to show the empathy, professionalism, or judgment expected in dementia and hospice care. Families should ask tough questions about staffing, cleanliness, and hospice coordination before trusting Brookdale Westlake Village with a loved one’s care. We wish we had.
