My grandmother has been a resident at this facility for over five years, and until recently, we have been generally satisfied with her care. Unfortunately, the quality of care has noticeably declined over the past several weeks.<br><br>After a recent fall, she is no longer able to transfer herself from her bed to her wheelchair and now depends entirely on staff for assistance. On numerous occasions, she has told our family that she has waited anywhere from an hour to an hour and a half after pressing her call button just to be helped out of bed so she can use the restroom. No resident should have to wait that long for such a basic need.<br><br>I have personally spoken with the Director of Nursing multiple times about these concerns. I was assured that things would improve and that staff would begin checking on her every hour. Based on what my grandmother continues to report, those hourly checks do not appear to be happening consistently.<br><br>We have also noticed significant changes with the food service. The meals were never exceptional, but they have become noticeably worse. Breakfast, which was typically served around 7:30 or 8:00 a.m., is now often not served until around 10:00 a.m. Lunch, which used to arrive between 11:30 a.m. and noon, is now frequently served after 1:00 p.m. She has also told us that menu options have become limited and that foods she routinely requested, such as fruit, are often unavailable.<br><br>It is heartbreaking to see someone who has lived here for five years experience this decline in care. Waiting over an hour for assistance to use the restroom is unacceptable and raises serious concerns about resident care and dignity.<br><br>Our family sincerely hopes these issues are addressed immediately. We are now considering installing a camera in her room so we can better monitor her care, and if meaningful improvements are not made, we will have no choice but to begin looking for another facility.<br><br>We are sharing this review because we hope management takes these concerns seriously. Every resident deserves timely assistance, proper nutrition, and to be treated with dignity and respect.

It’s a clean and well kept facility. Some of the folks work harder than others taking care of your love one. Meaning some do the minimum and some provide the extra attention when needed. So I rate them a C+. My opinion.

★☆☆☆☆ Avoid this facility at all costs.<br><br>My grandmother experienced horrific care and clinical neglect during her time at Bella Terra. As a Registered Nurse with 15 years of experience, I was deeply appalled by the unsafe conditions, structural hazards, and the complete lack of basic nursing standards at this facility.<br><br>During my visits, I discovered severe clinical violations:<br><br>Aspiration and Choking Risk: The staff left my grandmother in bed completely unable to speak because her mouth was crammed full of unswallowed medications mixed with applesauce. Leaving a resident with an uncleared mouth is a massive choking hazard. No one had checked to ensure she safely swallowed her medication; I had to personally intervene to clear her airway and help her swallow.<br><br>Gross Neglect of Hygiene: Her basic daily care was entirely ignored. I found her toothbrush still sealed in its original manufacturer packaging, proving oral care was not being done. She was also left in the exact same dirty shirt two days in a row, which I have photographic proof of.<br><br>Compromised Safety: Her call light was left completely out of reach while she was in bed, leaving her with absolutely no way to call for help while in a vulnerable state.<br><br>On top of the severe medical neglect, the physical environment of the facility was hazardous and falling apart. Her room featured a broken dresser, half-exposed electrical wires, and a wall with completely exposed drywall. This environment was unkept, unacceptable, and outright dangerous for elderly residents.<br><br>I reached out to the Director of Nursing multiple times to address these issues and was completely ignored for nearly a week. Because of the facility's total lack of accountability and the severity of these violations, I have already filed formal reports with the State of Illinois Department of Public Health (IDPH) and the Illinois Board of Nursing.<br><br>She is no longer at this facility, and I strongly warn others: do not trust this place with your loved ones.

Worst place ever. It’s like a third world place. Nothing like the photos and reviews. My mom’s first night she fell and ended up in the er. They messed up her meds they were not giving her what she was suppose to be taking. I had to meet with them to ask for a list of her meds and she was supposed to get a shot to produce red blood cells. In the two weeks there she was in er 3 days . Her dialysis wasn’t started on time meds not given . She couldn’t go to the bathroom and was constipated they just gave her Tylenol. She was complaining of stomach paint and yelling in pain. I made the night nurse call the doctor to see what could be done. He didn’t know his left from right I had to tell him. The staff doesn’t care they were gossiping about patients and families at the desk while I was waiting on a doctor. My mom’s first night ended up in er with dehydration and other issues. If you are not with your person all day and night they do not get taken care of. We brought my mom home with hospice and she passed away . No follow up from Bella Terra to see where she is. I called about picking up her items and the person took my name and number and of course no call back. Why am I not surprised. They prob dumped her stuff. That’s how they care for their patients and families . They just take money nothing else .
